We are looking for a Patient Care Coordinator to join our team and provide excellent patient care. The successful candidate will be responsible for coordinating patient care services, scheduling appointments, providing administrative support, and communicating with patients and their families. The ideal candidate will have excellent organizational skills, the ability to multitask and prioritize, and a strong commitment to providing quality patient care.

Responsibilities:

• Provide direct patient care and assistance.
• Coordinate patient care services with other healthcare providers.
• Develop and implement patient care plans.
• Monitor patient progress and evaluate outcomes.
• Assist with scheduling appointments and follow-up visits.
• Provide patient education on healthcare topics.
• Maintain patient records and document all patient care activities.

Job Type: Full-time

Pay: $17.00 - $19.00 per hour

Expected hours: 40 per week
