Greater Flint Health Coalition is Hiring an Operations Manager Near Flint, MI

Overview

The Greater FlintHealth Coalition is seeking a licensed social worker with clinical anddatamanagementexperience toserve as theOperations Manager for the Genesee CommunityHealthcare AccessProgram. The Manager willassist in the administrationof theMid-MichiganCommunityHealthcare Access Program (CHAP).CHAP provides medical home improvement,elevated blood lead level nurse case management,and family support coordination services forclients.

Thismanagerwill coordinate and overseeCHAP efforts globally to measure and exhibit programand service delivery outcomes at the client, practice, and system levels. The Manager will overseethe CHAP team’s data collection and analysis, program evaluation, and training specific to datasystems and collection protocols.Themanagerwill ensure that data and practice engagementrelated program procedures, practices,and community linkagesdocument the impact of servicesto addresssocial determinantsof health to achieve medical home connections and health equityfor individuals and families.The manager will support in the development of new activities,programs and services related to care coordination.

Mid-MichiganCHAP is an innovative and collaborative approach to improve the health outcomesfor Genesee County’spopulationwith Medicaid while reducing costs. ThroughMid-MichiganCHAP, a multi-disciplinary, community and clinically-based teamwill linkfamilies with a medicalhome (primary care physician), health behavior and asthma education, transportation to medicalhome visits, interpreting services, and community resources to address social determinants ofhealth.Mid-Michigan CHAPwill not provide direct health care or clinical services but serves as acommunity-based mechanism to eliminate barriers and improve coordinated use of services.Operated via the Greater Flint Health Coalition, theMid-Michigan CHAPOperationsManagerwillbe a member ofour team based in Flint, Michigan, working in a fast-paced, demanding, butrewarding collaborative environment.The Managerwill have the opportunity to useher/hissocial work experience, as well as clinical and community knowledge, to improve healthoutcomes and healthcare access for children.

Primary Responsibilities:

  • Together withtheSenior Program Directorand other members of the CHAP managementteam, is accountable forday-to-dayCHAP operations and service deliverythatdirectlyimpacttheprogram'sgoalsandobjectivesthroughtheplanning,delivery,documentation,and directing of CHAP resources.
  • Assists in executing the strategy to grow the Mid-Michigan CHAP program
  • Facilitates new partnerships and establishes newrelationships with referral partners.
  • Providesoversightofcontractualagreementswithfundingpartnerstomaintaincompliance with obligations.
  • Together with the Program Assistant, ensures the reporting production scheduledeadlines are met.
  • Engagesincontinuingperformanceimprovementactivitiesandprofessionaldevelopment to maintain and enhance clinical and programmatic knowledge.
  • Works closely withnurse,social worker, family support specialist, and community healthworker team memberstoidentify and develop methods forthe timelytracking andreportingofpertinent data so it can be used as programmatic information. Shapes andstandardizes methods for input, management, reporting,and use of data.
  • Serves as an advisor to CHAP Assistant Project Managers, Analysts, andthose whoconceptualize and operationalize programservice or outcomesdata.
  • Functions as a program evaluation leader who develops source of truth logic andmethodologies to be used for evaluation processes. Worksclosely with management andsupervision internally and with community partners to ensure sufficient, timely evaluativedata can be accessedand used.
  • Along with program supervision, ensures the timely completion of all applicable staffassignmentsandgoalattainmentaswellasdocumentationandotherrequiredpaperwork.
  • Participates in key committees ofMid-Michigan CHAP, specifically the CHAP PracticeManagers Subcommittee and CHAP Providers Subcommittee, and those of externalpartners as assigned.
  • Engages in applicable administrative, contractual, licensing and business activities toaccomplish defined goals and objectives toward the achievement of the program mission.
  • Works effectively with other members of the program team in the oversight of CHAPservices and operations, including case management, outcomes monitoring, patientreferral acquisition, community relations and other direct services.
  • Represents the program within the Greater Flint Health Coalition and at the community,state and professionallevels as requested.
  • Adheres to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) andall related legislation to safeguard Protected Health Information (PHI).
  • Serves as a resource for the community onCHAPprogramming and other EBLL issues.
  • Accepts other responsibilities as assigned.

Qualifications & Experience Required:

  • Bachelor’s degreein health orbusiness-relatedfieldrequired, Master’s preferred.
  • Minimum 2 years related experience.§Experience working in social work anddata managementor health services settingpreferred.
  • Possess the knowledge, skills and experience necessary to provide effectivedataand casemanagement and short-term solution-focused engagement, including basicmotivationalInterviewing techniques.
  • Knowledge/experience of healthcare systems, and home visiting programs stronglydesired.
  • Excellentcommunicationandinterpersonalskillsneededtoprovideeffectivecollaboration withinternal andexternal entitiesacross the continuum of care.
  • Knowledge of local community resources.
  • Must be able to work as a part of a team and in a collaborative setting.
  • Ability to accept and act upon constructive feedback and verbal or written direction ofwork.
  • Ability toexercise discretion and independent judgment with respect to matters ofsignificance, and perform work related to client complaints, disputes, and grievanceresolution.
  • Computer skills include a working knowledge of Outlook, Word, Excel, and databasesystems.
  • Demonstrates self-directed, self-motivated, responsible behavior.
  • Able to think independently and make sound judgments.
  • Maintains high level of confidentiality and is non-judgmental.
  • Demonstrates a high level of cultural competency in the daily performance of duties.
  • Informs supervisor of challenges; seeks guidance as needed.
  • Supports and cultivates self and fellow team member’s growth and development
  • Assumes responsibility of continuing education and improvement to enhance skills.
  • Ability to prioritizeand manage multiple priorities and projects while meeting deadlinesand expectations.
  • Ability to discuss and understand the issues that surround health equity and health caresystems.
  • Follows all federal, state, and local laws/regulations and Greater Flint Health Coalitionpolicies and procedures.
  • Considerate, attentive,and punctual.
