JOB TITLE: Part-Time Therapeutic Group Home Therapist/Counselor
SUPERVISOR: Director of Clinical Services
GOAL: To provide high quality, trauma-informed therapeutic care to youth residing in Gracehaven Therapeutic Group Home and use evidence-informed individual, family, and group therapies and curriculums.
KEY OBJECTIVES
  • Provide individual, group, and family based clinical services to youth who are at risk and/or have been exploited and are active clients engaged in Gracehaven Therapeutic Group Home (TGH).
  • Assess youth who are admitted to the TGH and make treatment and level of care recommendations in accordance with diagnosis and medical necessity.
  • Develop and implement individualized, therapeutic treatment plans in accordance with the youths diagnosis and treatment recommendations. 
  • Participate in a clinical on-call rotation to provide crisis management consultation to the Gracehaven TGH Service Team, conduct on-site, in person, screenings & risk assessments, as well as crisis intervention services to clients when deemed necessary. Note these services can also be provided via telehealth when appropriate.
  • Work in conjunction with Gracehaven Director of Clinical Services to implement and maintain QRTP evidence based treatment model & therapeutic interventions services.
  • Proactively contribute to and foster a trauma-informed & responsive organizational culture.
  • Maintain all compliance standards.
  • Actively participate in supervision & ongoing professional development- training to foster the development of clinical skills, provide best practice and enhance Gracehaven therapeutic services for youth & families who are at risk and/or have been exploited.
SPECIFIC DUTIES
Responsibilities include, but are not limited to, the following:
Clinical Responsibilities:  
  • Referrals: Assist in reviewing TGH referrals (i.e. ICCAs, clinical assessments, Ohio Comprehensive CANS or other LOC tools, etc.) and providing clinical placement consultation to the Director of Therapeutic Group when needed.
  • Establish therapeutic rapport and relationships with Gracehaven Group Home clients and families.
  • Assessment: Responsible for developing a comprehensive clinical formulation based on the completion of a diagnostic assessment, to establish medical necessity for services at time of admission. The comprehensive assessment will include at minimum an interview with the client, review of records, observation of current level of functioning, the formulation of a diagnostic impression, level of care, and individualized treatment recommendations.
  • Treatment Planning: Facilitate the collaborative development of an Individual Service (ISP) - Treatment Plan with the client, family, and other service providers. Monitor implementation of this plan. Regularly review, update/revise service recommendations/treatment goals, intensity of services based on the ongoing assessment of clients progress and needs, and maintain medical necessity of this plan with the interdisciplinary team according to regulatory guidelines (ODJFS-QRTP, OhioMHAS, etc.).
  • Therapy: Responsible for providing behavioral health services, including individual, group, and/or family therapy to adolescents and families based on mutually agreed upon goals with the client and/or parents as identified in the ISP. - Treatment Plan.
  • Discharge Planning: Participate in monthly Multidisciplinary Progress Review and Discharge Planning Meetings and serve as an advocate for the clients to ensure that appropriate services are provided and permanency/maintenance planning is prioritized.
  • Documentation: Provide accurate and timely documentation of service delivery and client’s progress, in accordance with the treatment goals and within the timelines set forth by the agency and regulatory guidelines.
  • Maintain effective, timely and thorough verbal and written communication with clients, families, referring agencies, and other service providers as described in the interdisciplinary team process.
  • Clinical On-Call: Participate in a clinical on-call rotation to provide 24/7 hour clinical crisis management services for active group home clients, including in person and/or telehealth (as appropriate) risk screening and assessment (e.g. suicidal, homicidal, psychotic, etc.) and crisis intervention services. Identify clients with urgent/emergent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities when appropriate. 
  • Assist the client and Gracehaven service team in developing effective safety plans and behavioral intervention plans as needed.
Trauma Informed-Responsive Organizational Culture Requirements:
  • Supports the philosophy, goals, and mission of Gracehaven.
  • Shares information and training resources with staff that support the implementation of trauma informed/responsive practices when interacting with clients, families, and service team members.
  • Supports a trauma informed and responsive ethos at the group home by modeling, coaching, and/or reinforcing the teaching of symptom management, including relaxation techniques, anger management, therapeutic goal-setting, problem solving, coping skills, etc.
  • Utilizes therapeutic, trauma informed/responsive intervention techniques before, during and after times of crisis to support safe client de-escalation and stabilization.
Compliance Responsibilities
  • Maintain an active license that is in good standing with the state of Ohio Social Work and Counselors Board.
  • Obtains and maintains Dialectical Behavioral Therapy Certification, Gracehaven Level II QRTP approved Trauma Model.
  • Participates in professional development (e.g. training, workshops, conferences, etc.) to maintain professional knowledge and proficiency, as well as the Continuing Education Units (CEUs) necessary to maintain appropriate licensure and credentialing. 
  • Clinical services must be provided in accordance with all policies and procedures, and current federal, state, and local standards, guidelines, and regulations governing Gracehaven Therapeutic Services.
  • Provide therapeutic services in a manner that ensures confidentiality of the client is maintained and HIPPA guidelines are followed.
  • Maintains accurate and timely documentation/outcomes data/records and service delivery requirements, as set forth by the agency policy & procedures, as well as regulatory bodies. 
  • Maintains productivity standards for position.
  • Participates in program quality improvement activities and maintains quality assurance standards.
  • Strives to maintain positive client and stakeholder satisfaction.
Supervision/Team Responsibilities
  • Attends all COYFC/Gracehaven training and staff meetings.
  • Assists in all Gracehaven Fundraising Events.
  • Participates in supervision, case review staffing and other intra-agency meetings.
  • Performs all other duties as assigned by the Director of Clinical Services.

  • Pray for and share spiritual content with existing and prospective donors to COYFC.
Minimum Qualifications:
  • Demonstrated skills in managing care to children and families, with experience in providing care to individuals impacted by trauma, especially sexual violence and trafficking, and their families.
  • Excellent communication or interpersonal skills to establish and maintain positive relationships with clients, their families, and multidisciplinary professionals/service providers.
  • Education: Master’s degree in social work/counseling or a related behavioral sciences field from an accredited university or professional school required. Note: MSW and LPC Interns under the direct supervision of an LISW-S or LPCC-S may be considered for this role.
  • Licensure: LSW or LPC issued from the State of Ohio Counseling and Social Worker Board, or is license eligible and willing to obtain, required; LISW or LPCC (Independent License) preferred; LISW-S or LPCC-S (Supervisory License) preferred.
  • Certifications: Level II or III QRTP approved Trauma Model certified is preferred; or willingness to obtain required.
  • Certification: Ohio Comprehensive Child and Adolescent Needs & Strengths (CANS) Certified or willing to obtain certification.
  • Experience: Two (2) or more years providing trauma informed counseling to youth and families is preferred.
  • Clean background check
  • Agreement with the Central Ohio Youth for Christ and Gracehaven Standards of Conduct and Statement of Faith.
