Gracehaven is seeking a compassionate and dedicated Community-Based Case Manager & Clinician to join our team. You will provide trauma-informed clinical services, case management, and support to youth, families, and the broader treatment team.

Responsibilities:

• Individual and family therapy
• Crisis intervention and safety planning
• Comprehensive assessments and treatment planning
• Collaboration with families, community partners, and other providers
• Client advocacy and discharge planning
• Maintaining accurate documentation and meeting quality assurance standards

Qualifications:

• Master's degree in Social Work/Counseling or a related field
• LSW or LPC license in Ohio (or eligible)
• Experience working with trauma, especially sexual violence and trafficking
• Excellent clinical and communication skills
• Agreement with the Central Ohio Youth for Christ and Gracehaven Standards of Conduct and
  Statement of Faith.
