Overview

The Social Worker provides all clinical and therapeutic services for youth in a residential placement environment and their families. The Social Worker works to insure timely community re-entry, permanency, safety and well-being for each youth assigned to their case load. Participates in the Sanctuary Model of care to insure trauma-sensitivity and supports Missouri model of group treatment.

Location: 280 Shepherd Avenue, Brooklyn, NY 11208

Hours: 35 Hours/Week; Exempt

***Salary Range 58K-$69,227 Annually***

Major Duties:

  • Perform all case management and social work services in collaboration with program case planner and in compliance with mandates. Collaborate with case planner in preparation and submission of all needed court reports.
  • Conduct social work intake within specified time frames, including administration of required assessments and development of psychosocial.
  • At admissions, begin the process of identifying significant adult connections who can serve as a possible discharge or visiting resource, and formulating discharge planning, including through the development and use of a genogram.
  • In conjunction with Health Services, coordinate mental health related activities including but not limited to acquiring mental health history documentation, facilitating evaluation/treatment referral, following up with treatment provider about progress, etc.
  • Provide therapy: individual, group, and family counseling, using agreed-upon models including TF-CBT.
  • Work a schedule to be agreed with supervisor and that contains at least one late day a week. Be available via agency phone at weekends for crisis intervention as needed.
  • Function as member of multidisciplinary treatment team. Participate in Team Meetings, Treatment Team Meetings, Service Plan Reviews, Family Team Conferences and other meetings.
  • Act as a liaison with Division of Youth and Family Justice (DYFJ) of ACS, family court and appropriate community agencies and other stakeholders.
  • Facilitate community re-entry and in collaboration with other team members arrange for wrap-around services in the community as needed, collaborating with all aftercare services.
  • Collaborate with team members to prioritize permanency planning and develop concurrent plans.
  • Responsible for maintaining current electronic and paper-based case record, including initial treatment plans, monthly treatment plans, progress notes, and Psychosocial Assessments. Learn and use Connections statewide computer system to gather, share and submit all case information to appropriate ACS and OCFS units and other voluntary agencies within given time parameters, and use other databases as required. Contribute to FASPs, permanency reports and other documents as needed.
  • Support the Missouri model of group treatment.
  • Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care in the department/program to create a non-violent and trauma-sensitive culture.
  • Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture.
  • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and to create a non-violent and trauma-sensitive culture.
  • Perform other duties as assigned.

Qualifications:

  • Master’s degree in Social Work and licensure (LMSW) required.
  • Clinical work with youth and families required; including crisis management.
  • Proven participant sensibility and the ability to communicate with staff on all levels.
  • Demonstrated competence in verbal, written, organizational, and prioritization skills.
  • Capability to transition easily between working independently with a high degree of self-initiative and contributing as part of a team.
  • Excellent crisis intervention, advocacy and problem solving skills.
  • Must be able to use public transportation systems, including subways and busses to attend various meetings and make home visits.
    • Must attend and pass mandated Safe Crisis Management (SCM) intervention technique trainings.
    • Bilingual English/Spanish preferred.
    • Must have the ability to work a flexible schedule, including evenings, weekends and holidays, as needed.
  • Must have a commitment to work from a strength-based, youth and family development and trauma-informed perspective.
