Social Worker
The Chateau at Brooklyn Rehabilitation & Nursing Center is seeking a full-time Social Worker for our skilled nursing facility. The ideal candidate will have experience in long-term care/ sub-acute setting and must have previous healthcare experience.
Social Worker Duties:
- Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotion needs of the resident
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Social Worker Requirements:
- BSW or MSW preferred, or Bachelors degree in the related field
- At least 3 years experience in the Social Work field required
- Ability to work flexible schedule
- PCC experience preferred
- Long Term Care / Subacute Experience Required
Salary Up to $65,000 a year
