Social Worker Responsibilities
- Ensure that themedical, social, and emotional needs of each resident are provided for
- Work together withother staff to identify potential difficulties for residents
- Serve as a residentadvocate
- Provide crisisintervention, counseling, or other services to residents and families
- Assist new residentsand families in the initial adjustment phase
- Work with residentsand families on planning discharges
- Act as a liaison toFederal, State, and local agencies providing service to the resident community
- Maintain a productiveworking relationship with other department supervisors
- Coordinate socialservices to assure the daily social services can be adequately provided
- Bachelor's degree inSocial Work

- Previous experience ina supervisory capacity in a skilled nursing facility
