Position Description

The Intake Specialist serves as the initial contact at GMHC for incoming clients. The Intake Specialist will conduct comprehensive psychosocial interviews, with new clients and existing clients in need of services. The Intake Specialist will refer clients to appropriate services within the agency.

Essential Job Functions

The following duties are mandatory requirements of the job:

• Conduct psychosocial interviews with clients who wish to receive services at GMHC, using Intake and Assessment tools provided
• Document in TREAT data collected from assessments
• Meet the concrete needs of clients presenting with single and/or multiple-issue problems, such as the completion of intake documents or request documentation from medical provider
• Provide client with internal and external referrals once intake has been completed
• Ensure group dashboard and client time tracking data are up to date and accurate

The following duties are to be performed as assigned by the supervisor:

• Ability to multi-task and prioritize tasks with competing deadlines
• Ensure referrals and informational resources are up to date and reflect relevance
• Ability to work across multiple grants and meet deliverables as assigned
• Complete other assignments as assigned by supervisor

Education and Certification

• High School Diploma, GED, or its educational equivalence

Special Skills and Knowledge

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:

• Knowledge of Microsoft Office, Outlook and Excel
• Strong written and verbal communication skills
• Bilingual in English and Spanish preferred
