JOB SUMMARY:

Provides health care services through delivery of professional nursing care appropriate to patients’ age and condition as documented in the nursing and medical plans of care and in consideration of the diverse physical, emotional, spiritual and socio-economic needs and beliefs of families and the community in accordance with all Tribal, state, federal and licensing standards and guidelines

MINIMUM QUALIFICATIONS:

1. Current State of Arizona RNlicense; American Heart Association BLScertification within 30 days of hire

2. Must have at least 1- 2 years previous nursing experience with ability to perform more advanced clinical skillsassociated with renal dialysis; Hemodialysis certification preferred

WORKING ENVIRONMENT (PHYSICAL & MENTAL DEMANDS):

§ Activities are performed in an environmentally controlled dialysis center subject to frequent periods of standing, walking, stooping, pulling, pushing, bending and lifting objects of varying weights up to and including 150 pounds

§ Activities performed require ability to regularly assist patients to and from various types of equipment

§ Use fine motor skills in the administration of health care related activities, manipulation of a computer keyboard, mouse and visual acuity for computer monitor

§ Regularly works in an environment subject to high stress levels, exposure to communicable diseases, fumes, chemical, noise and blood born pathogens and frequently changing priorities

EQUIPMENT USED:

§ Telephone, computer (monitor, keyboard and mouse), medical instruments (including, but not limited to: disposable sharps, blood pressure cuff, stethoscope, dialyzer, scale and thermometer, fax, printer and refrigerator

Scope of Supervisory Responsibilities:

Provides clinical supervision and oversight of other licensed and non-licensed health care providers Receives review of completed objectives to ensure achievement of established goals’

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Completes nursing assessment of patient’s condition consistent with age and condition including physical, psychological, social, developmental and educational needs

2. Determines initial level of compliance with treatment plan in accordance with hospital policy and unit guidelines

3. Performs ongoing patient/family assessment and identifies variances from diagnosis

4. Maintains and updates patient’s medical records through concise, accurate and timely documentation

5. Works collaboratively with patient care team and family members to establish and implement plan of care to ensure coordination of activities and ongoing communication of patient health status

6. Participates in patient care rounds and patient care conferences as appropriate

7. Continually assesses and evaluates patient’s health status and response to care, teaching and discharge plan

8. Shares observations and assessment outcomes with health care team and family to ensure communication and understanding of patient health status, progress and ongoing needs

9. As primary nursing care provider, supervises patient care activities of assigned team members, delegates activities appropriate to knowledge, skill and competency; provides timely feedback and review of care delivery to ensure maintenance of patient safety, infection control procedures and therapeutic environment

10. Maintains professional and technical competency through regular participation in a variety of educational programs and in-services, reviews relevant professional publications and materials

11. Develops and maintains good working and professional relationships with other health care team members and providers to facilitate open and cooperative communication

12. Provides orientation to the unit and hospital systems and processes to patients and family members to ensure familiarity with unit policies and requirements

13. Contributes to the team/unit’s efforts by completing required activities within established parameters to ensure compliance with all applicable medication, storage and documentation standards, regulations and requirements

14. Reviews and reinforces with patient/family ongoing education of basic technical procedures, activities and safety necessary for daily living

15. Participates in departmental orientation, on the job training and quality assurance programs/initiatives

16. Participates in a variety of department and hospital educational programs to maintain current skills and competency levels; identifies and discusses performance or training needs with Supervisor

1. Participates in a variety of hospital and/or unit based committees and programs

KNOWLEDGE, SKILLS AND ABILITIES:

1. Knowledge of the principles, practices, techniques and methods of patient assessment and health care advisement in accordance with nursing practice standards

2. Knowledge of the practices, methods and techniques to provide care or services appropriate to pediatric, adult and geriatric renal patients served

3. Knowledge of the principles of growth and development over the life span

4. Knowledge of principles, practices and procedures related to immediate and ongoing renal assessment, re-assessment and implementation of appropriate nursing interventions

5. Knowledge and understanding of the physical, mental and emotional requirements of specific job functions, activities and patient care delivery requirements

6. Knowledge and understanding of regulatory agency reporting requirements and standards

7. Knowledge of human behavior during stressful situations in order to exercise emotional control, tact, patience and reliability in carrying out patient care

8. Knowledge of the proper and safe handling, use and disposal of medical equipment, instruments and sharps

9. Knowledge of the principles, practices, methods and techniques of quality improvement initiatives, root cause analysis

10. Knowledge of pharmaceuticals to recognize desired effects, side effects and complications

11. Knowledge and respect for varying socioeconomic, cultural and belief systems

12. Ability to instill and foster trust in others through consistency in work methods, behaviors and follow through

13. Ability to accurately and reliably interpret data for determining health status and initiate appropriate action in emergency situations

14. Ability to assess family dynamics for well being of patient by recognizing their limitations and strengths through completion of thorough and effective interviews

15. Ability to perform various diagnostic and therapeutic procedures according to standards, training and certification/licensure

16. Ability to apply active listening and critical thinking skills for problem identification and resolution

17. Skills associated with making diagnosis, choosing, initiating and modifying therapies and providing emergency treatment

18. Skills associated with effective listening, communication (verbal and written) and collaboration

19. Skills in performance of technical procedures according to established standards and training

20. Ability to regularly change priorities to accomplish tasks with frequent interruptions

21. Ability to clearly communicate verbally, read, write, see and hear to perform essential functions

Pay: $35.00 - $49.00 per hour
