Responsibilities:

• Provide primary care services and manage acute and chronic health problems using evidence-based protocols
• Provide health screenings, immunizations and other disease prevention programs
• Provide age-appropriate assessment and interventions in order to meet the needs of patients and/or families served
• Provides patient/family education, health counseling, and health advocacy services to patients and/or their families
• Collaborate with community health nurse on care and follow-up

Schedule:

• 3, 4 or 5 day work week options
• 8a-5p

Salary: $126-136k

Qualifications:

• Covid Vaccination is REQUIRED
• 2 years of NP experience
• Bilingual Spanish, preferred
