POSITION SUMMARY

 

This position will be responsible for facilitating Youth Centered Reentry Team process for youth in their assigned facilities. They will engage youth, families, staff, and community partners in the reentry process with planning for a successful return to the community.

RESPONSIBILITIES AND DUTIES

 

·Manages an assigned caseload of youth in assigned long term YDC and/or youth screened for YDC held at RYDC and are 120 days from release in accordance with policy 25.1.

·Works closely with team of professionals to determine the needs of each youth and coordinates a range of support services.

·Initiates the Domains of Aftercare (DOA) module in JTSnx within three days of youth’s initial YCRT meeting and will update the DOA as needs change.

·Reviews and updates the Reentry checklist in JTSnx within three days of each YCRT meeting.

·Ensures that eligibility for early release and step-down placement is discussed as applicable.

·Includes Operations Support Manager in YCRT meetings for youth being released to residential placement.

·Facilitates and schedules all Youth Centered Reentry Team Meetings (YCRT).

·Documents progress of Youth Reentry Checklist and service planning goals.

·Maintains and updates information on the 150-day release report, bi-weekly.

·Provides a setting where youth and family’s needs are a priority and establish agreed upon services, supports, and resources.

·Utilizes a statewide database to connect youth to services when there is a need.

·Works with CCM to ensure that transition plans are initiated after the initial YCRT and will provide updates to the plan after 120- and 90-day meetings for successful collaborative effort with case manager and reentry staff.

·Provides support to groups or programs initiated by ORS.

·Increases family participation for The Chat monthly call, YCRT meetings and other opportunities as they arise.

MINIMUM QUALIFICATIONS

 

Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; or Two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or equivalent position.

 

PREFERRED QUALIFICATIONS

Preference will be given to applicants who, in addition to meeting the minimum qualifications, possess two or more of the following:

·Master's Degree in Education, Counseling, Social Work, or Criminal Justice

·Minimum two years youth/ adolescent case management experience; Knowledge or prior experience working with justice involved youth or adults

·Exceptional written and oral communication skills; mastery with meeting facilitation

·Experience in reentry/aftercare for youth or adults.

PHYSICAL REQUIREMENTS / WORKING CONDITIONS

 

Working Conditions:

·Normal office environment

 

Physical Demands:

·Statewide travel periodically as required.

·Moderate lifting as required up to 30 pounds
