Georgia Harm Reduction Coalition is Hiring a Lead Linkage Specialist Near Mountain, GA

Under the supervision of the Linkage Manager, the Linkage Team Lead will supervise over a team of specialists, as well as perform duties not limited to linkage to HIV and HCV medical care, linkage to essential services such as housing, linkage to supportive services such as transportation and substance use treatment facilities, counseling for substance use disorders, and delivering individual and group interventions. This position will require to attend outreach, events, and attend local, regional and national training.

Responsibilities Include, but not limited to:

Supervision Duties

  • Supervises and supports a team of Linkage Specialists with linkage, counseling, and delivery of individual and group interventions.
  • Leads weekly meetings with team to discuss and set goals associated with organizational metrics for department programs including but not limited to Linkage, Project SAFE, Youth Impact Project, and Mothers On Move; monitor metrics and ensure that the team is meeting program deadlines and goals.
  • Monitors team’s productivity and provides constructive feedback and coaching through individual monthly sessions.
  • Ensures success of all program deliverables
  • Supervises staff properly charts client retention navigation progress and reporting through data entry in data gathering systems (Evaluation Web, SPARS, EHR, and more).
  • Communicates daily with Program Manager ensuring to update on program reports, employee relations, and other topics relating to the program.
  • Provides and/or finds coverage for client appointments and shifts when needed.

Additional Duties

  • Provide case management to patients living with HIV and HCV including identifying clients needs and insurance status
  • Assists in the coordination of healthcare services and provider referrals through linkages to primary HIV and HCV healthcare and assists with retention in care by linkage to supportive services.
  • Maintains a caseload of clients enrolled in a strength-based case management and motivational interviewing intervention and follows the client for a period of 4-12 months (depending on the program) to ensure retention in care.
  • Utilizes strengths-based intervention to link clients back into care.
  • Provides transportation for clients to and from appointments.
  • Charts patient retention navigation progress and reporting through data entry; maintain client files according to program protocols.
  • Ensures timely and accurate data entry and reporting.
  • Reviews and prepares weekly outcome reports of all grant deliverables for Program Manager
  • Ensure all data are entered accurately in to SPARS and Evaluation web
  • Represents the organization at community events and meetings when assigned
  • Adheres to strict procedures of confidentiality, release of information and data security
  • Performs additional duties as assigned by the supervisor

Required Education & Experience:

  • Bachelor’s degree in Social Sciences, Healthcare, or a related field of study, preferred
  • Team supervision/managerial experience, preferred
  • HIV case management including, completion of Ryan White / AIDS Drug Assistance Program (ADAP) applications
  • 2 years of qualified experience; or an equivalent combination of training, education and experience that demonstrates the ability to perform the duties of the position
  • Must have reliable access to a vehicle and a valid driver’s license
  • Strong ability to delegate tasks and collaborate effectively in a team environment
  • Experience related to Housing and substance use policies and programs training
  • Experience working with providers in linking clients to care
  • Experience in navigating insurance and/or patient assistance programs
  • Spanish Speaker, preferred

Required Skills & Abilities

  • Computer literacy with strong knowledge of Google Drive and Microsoft Office
  • Proven ability to maintain strict confidentiality and make sound decisions in crisis situations
  • Ability to communicate effectively and clearly, both verbally as well as in writing.
  • Displays strong customer service skills with the ability to interact with clients in a professional, calm, patient, and friendly manner.
  • Possesses strong time-management skills with the ability to meet deadlines under pressure while handling multiple projects
  • Excellent organizational skills with attention to detail when processing paperwork
  • Strong ability to accurately enter and report data
  • Highly motivated with a strong work ethic and the ability to work both independently and in a group environment.
  • Ability to work in a high-stressed environment and interact with marginalized individuals and communities.
  • Ability to work late night shifts if necessary.
  • Displays respect for peers, supervisors, and clients.
  • Team supervision/managerial experience
  • HIV case management including, completion of Ryan White / AIDS Drug Assistance Program (ADAP) applications
  • Spanish speaker

Physical Requirements:

  • Must be able to lift up to 20 lbs at times
  • Participation in community outreach which involves prolonged periods of standing (Community Outreach initiatives occur year-round)
  • Ability to travel as needed to partnering agencies (personal transportation required).
  • Ability to adjust to flexible schedules as published by the department manager.
  • Ability to work efficiently in adverse weather conditions.
