Gardner Health Services is Hiring a Homeless Program Coordinator Near San Jose, CA

Gardner Health Services (GHS)is currently recruiting to fill the position of Homeless Program Coordinator. This is a full-time position based in Alviso and San Jose, California. Under the general supervision of the Chief Financial Officer, the Homeless Program Coordinator manages the operational aspects of GHS’s Healthcare for the Homeless Program. The position ensures the homeless of Santa Clara County have access to compassionate, professional and accessible healthcare; to include: medical, case management and counseling services and ensures that the fulfillment of requirements set forth by licensing and funding entities are met. The Homeless Program Coordinator builds relationships with outside government and service entities which are developed, fostered and met with long term engagement to ensure commitment to care for this underserved community. Responsible for the overall fulfillment of GHS’s operational objectives and the Department of Health and Human Services (DHHS) as it relates to the provision of homeless healthcare services and responsible for the organization and fulfillment of health fair requests from the community.

HEALTHCARE FOR THE HOMELESS PROJECT

Our medical team provides a variety of preventive and primary healthcare services to homeless individuals including substance use treatment and counseling, case management, and urgent care referrals. The program also offers eligibility and registration services through homeless shelter locations in the Santa Clara County. Our services are provided by two medical mobile units, at John XXIII Service Center, and at two intermittent clinics in partnership with City Team and the Bill Wilson Center.

Minimum Qualifications:

The minimum qualifications of this position is listed below. An equivalent combination of education and experience may be considered.

  • Bachelor’s Degree, preferred; minimum of three (3) years’ experience in a supervisory capacity in an outpatient clinic program setting, community health care setting, or other complex homeless related service entity.
  • Demonstrate effective communication and interpersonal skills, skillful writing skills, including ability to write narratives for grant proposals, publications and presentations.
  • Multi-task capabilities, with ability to problem solve on complex levels.
  • Ability to work independently and exercise good judgment.
  • Experience managing a team of 12–16 staff.
  • Effective budget management skills (> $1.5M).
  • Participate in on-going committees within GHS and with outside agencies.
  • Interact in a courteous manner with patients, clients, and staff.
  • Work effectively with diverse populations by fostering a teamwork environment.
  • Schedule can be flexible as needed.
  • Must have reliable transportation.

Compensation:

Compensation and level of position placement will be determined based on relevant qualifications and experience.

Homeless Program Coordinator: $90,000 - $110,000/annually
