TAY Service Network Manager - Position Summary 

The Contra Costa County (CCC) Transition Aged Youth (TAY) Service Network aims to ensure that every young adult (ages 18-25) on probation gets access to services and programs that meet their needs and support them to achieve their goals.

The TAY Service Network is sponsored by the Contra Costa County Probation Department through state funds (SB 678) intended to reduce recidivism among individuals on felony probation, mandatory supervision, and/or post-release community supervision by utilizing resources and making investments in evidence-based supervision practices and treatment interventions. In doing so, the TAY Service Network is designed to establish a collaborative system of support that will include aligning TAY-specific probation caseloads with TAY-focused supportive services and ultimately building the capacity of both the Department and TAY Service Network partners in working with justice-involved young adults. 

Fresh Lifelines for Youth (FLY) serves as the Network convener (i.e. “backbone” agency) to support the development and implementation of the TAY Service Network design. In this role, FLY convenes Network collaborative meetings, Network steering committee meetings, and TAY-centered case conferencing. As the “backbone” agency, FLY manages and facilitates the referral process and service coordination for every referred young adult into the Network. They will also provide training opportunities to TAY-focused probation officers and TAY Service Network Partners to further enhance the collective delivery of services to a young adult population.

FLY is hiring a TAY Service Network Manager to oversee this body of work in Contra Costa County. This position is newly-created and will be responsible for managing the activities of the Network, developing and sustaining Network partnerships, and data management and reporting for the Network.

Position Responsibilities

  • Network Management

    • Coordinate the TAY Service Network meetings including scheduling, securing meeting locations, preparing meeting agendas, and ordering food. 

    • Facilitate monthly TAY Service Network meetings and partner with the COO to support the quarterly TAY Service Network Steering Committee meetings.

    • Manage and build out TAY Service Network communications with partners and young adults.

    • Coordinate and plan the TAY Service Network Training Plan

  • Network Partnerships

    • Meet regularly with Contra Costa County Probation to ensure alignment and problem-solve through challenges.

    • Support a group of compensated TAY Advisors for the TAY Service Network to contribute to meeting and training planning

    • Sustain partnerships with current TAY Network Partners, including system partners and community partners

    • Cultivate new partnerships for the TAY Service Network

  • Data Management & Reporting

    • Oversee the TAY Service Network Intake and Coordination of the referral process

    • Partner with the COO to implement the TAY Service Network Learning Agenda, including administering surveys to partners and youth adults and conducting quality assurance checks with Network Partners

    • Oversee Homeless Management Information System (HMIS) activities of the TAY Service Network, including attending relevant HMIS provider meetings and completing HMIS data entry 

    • Complete TAY Service Network contract reporting

Compensation & Benefits

This position is a full-time/part-time non-exempt/exempt role at a rate of $71,958/annually with differential pay options based on an advanced degree and, when applicable to the role, speaking another language. Our Total Rewards Package also includes the following:
