As a part of our continued success and growth, we are seeking qualified applicants for a Discharge/Care Coordinator position.

Key Job Duties and Responsibilities:

• Initiates discharge planning and arranges aftercare appointments/referrals.
• Completes all needed discharge paperwork with patients and supports at time of discharge.
• Completes psychosocial assessments on new patients within the required timelines.
• Engages in collateral contacts with community organizations, support persons, and other providers and appropriately documents the contacts.
• Attends appropriate team meetings as needed.
• Other duties as assigned.

• Bachelor’s degree from an accredited college or university in Social Work.
