Family Support Specialist (Bilingual -Spanish)

Division Description

Felton’s Children, Youth, Family, and Transitional Age Youth division offers comprehensive, early care and education, family support, and mental health services across San Francisco to over 400 families.
Via a two-generation approach, Felton’s Family Support services promote strong families and optimal development for their children by focusing on the Strengthening Families Framework. The primary function of the family supports The Five Protective Factors; parental resilience, increase social connections, increase knowledge of parenting and child development, providing concrete support in times of need, and supporting social-emotional competence in young children. This approach serves as a one-stop integrated service continuum, supporting families to address their needs largely under one roof, with a core group of staff with whom they can build trusting, supportive relationships.

ECE Programs

Felton Institute has been providing comprehensive Early Care and Education (ECE) services in San Francisco since 1971. Felton now has ECE programs in SF’s Mission, Bayview, and Visitacion Valley neighborhoods; and offers a school-readiness program for infants, toddlers, and preschoolers (2 weeks to 6 years old). Felton’s ECE programs include 4 infant, 7 toddler, and 11 preschool classrooms, which serve approximately 400 children, full-day/ year-round. These multicultural, educational classrooms provide a nurturing, developmentally challenging, and inclusive environment for very young children of low/moderate-income families. We are committed to inclusive, positive environments that build on children’s positive self-esteem and self-image.

Early Intervention and Inclusion Programs

To meet the needs of young children with delays or disabilities, Felton Institute provides an array of Early Intervention programs that are funded through the Golden Gate Regional Center (GGRC), including Speech and Language Therapy and our Felton Early Autism Program (FEAP). These programs are geared towards infants and toddlers who may be at risk or who have a developmental delay or disability. Additionally, Felton offers preschool inclusion services that include 1:1 specialized instruction, Speech and Language Therapy, case management, and social skills groups designed to meet the needs of preschoolers who have Individualized Education Plans (IEPs) or those who require the extra support.

Family Support Programs
The family support programming at Felton includes the Young Family Resource Center (YFRC), which is a population-focused family resource center that is peer-directed, peer-focused, to target teen and young adult parents, their children, and their families. The Teenage Pregnancy and Parenting Project (TAPP) is a city-wide program with comprehensive case management and educational services for pregnant and parenting teens. The Sunshine Community Center (SCC) and Solmar Community Center (SCC) offer parenting education, support groups, resource and referral services, a weekly farmer’s market and essential items distribution, including access to community services.

Brief Job Description
This Bilingual Family Support Specialist role will be specializing on Early Intervention services and is referred to as Early Intervention and Inclusion Family Support Specialist (EII-FSS) for the remainder of the Job Description. The Early Intervention and Inclusion Family Support Specialist (EII-FSS) will maintain a caseload of 25-30 families with children with special needs. The EII-FSS is responsible for support case management, care coordination and service navigation for families navigating the complex systems of Golden Gate Regional Center and San Francisco Unified School District. The EII-FSS is responsible for working with referred families from referral through start of services, and working with SFUSD referred families from referral through transition to SFUSD programming. For families served under GGRC, the EII-FSS works closely with the EII Director and EI supervisor around clinical appropriateness in the program, and with the Admissions team to complete the enrollment and subsidy paperwork.
The EII-FSS’s caseload will be comprised of current Felton ECE families, families who are receiving EI services through GGRC, and families who participate in Felton’s Developmental Playgroup. The EII-FSS is responsible for supporting with enrollment, completing Family and Child’s Needs Assessments, completing the Family Support Plan and completing Transition meetings for all families on their caseload. The EII-FSS is responsible for ASQ screening across the division and is the primary holder of the Sparkler initiative. The EII-FSS is responsible for enrollment and family support record-keeping, reporting, and monitoring is necessary to ensure that all program performance standards and regulations are fulfilled. This role requires sensitivity and considerable ability to use good judgment and critical thinking in making decisions in accordance with existing regulations and departmental policies and procedures. They are a liaison with both families and the public.

ESSENTIAL DUTIES & RESPONSIBILITIES:
Direct Family Support
  • Completes Family Needs Assessment with caregivers
  • Completes the Family Support plan with caregivers
  • Regularly follows up with families via phone, zoom, in person and over email regarding referrals and care coordination
  • Conducts tours with prospective families
  • Schedules and conducts intake meetings, family needs assessments, and all necessary paperwork involved in the enrollment process
  • Supports clients from start to finish with the enrollment process with SFUSD including supporting clients through their IEPs and IEP appeals if necessary
  • Provide family support referrals to families on your case load, to internal Felton and external community resources.
Data Collection, Report Writing and other Administrative responsibilities
  • Safeguard and maintain individual records to keep consumer files complete, confidential, and in order. This includes filing client documents in secure areas; adhering to the recommended organization of components of individual files; and obtaining and documenting Releases of Information agreements prior to sharing private consumer information with others.
  • Provide relevant information on SFUSD services for children with special needs, including the IEP process and other placements available in the community that may meet the needs of the child and family.
  • Using CIRCE and the Sparkler app to track data and share progress with both clients and the EII Leadership team
  • Update various databases with enrollment information on a weekly, monthly and quarterly basis.
  • Manages referrals and waitlists for GGRC funded programs
  • Works with EI Supervisor and EII Director to manage enrollment
  • Works with all teams to manage accurate/prompt billing and productivity requirements
  • Manages all necessary paperwork and documentation related to enrollment
  • Supports with service coordination with Outside Service Providers
  • Collaborates with other community-based organizations to connect families to resources
  • Documents outreach and recruitment efforts and follow-up with interested families.
  • Maintains organized and complete family files for each enrolled child, including all funding sources – Title 22 require that these be in one place. Codes labels of files so that California Department of
  • Education files can be easily identified for review.
  • For GGRC-funded children, maintains and organizes Purchase of Services and Individualized Family Services Plan (IFSP) documents.
Community Collaboration
  • Supports with transitions (e.g. from EI to Pre-K, from Felton to external providers, and when necessary, between ECE programs)
  • Manages referrals for SFUSD services
  • Maintains important records related to children’s IEPs
  • Supports with service coordination with Outside Service Providers
  • Collaborates with other community-based organizations to connect families to resources
  • Develops partnerships, acts as a liaison, to establish communication and rapport between community and agency, to publicize supportive services available, recruit volunteers, leverage service delivery, and coordinate referrals.
ASQ Screening:
  • In collaboration with other EII-FSS and EII Leadership, completes annual ASQ Screening for all ECE children using the Sparkler app
  • Ensures the 100% of ECE families have screenings completed on the ASQ
  • Works with the EII Leadership team to fully implement the Sparkler app
Other Responsibilities:
  • Demonstrate sensitivity and competence in dealing with ethnic, cultural, and socioeconomically diverse client population spanning over a large geographic area
  • Ensure that the integrity of the individual family's culture is fully integrated into every aspect of the care and support offered to the child and family
  • Participate in training opportunities to maintain individual and organizational competency in assigned areas of responsibility. This includes demonstrating competencies identified for certification as Family
  • Support Specialist and required Policies and Procedures; consistently updating knowledge and skills relevant to position responsibilities; and attending in-services, conferences, workshops, and required agency meetings as appropriate.
  • Participate in weekly individual and group supervision.
WAITING LIST, CLIENT ASSESSMENT, CERTIFICATION, AND MONITORING
  • Maintain ongoing and accurate up-to-date client records for the caseload of families that have children with special needs. This includes those who are receiving GGRC services, families who also receive subsidized childcare services at Felton, and those accessing family support services.
  • Track and document time and services with accuracy to ensure services are provided according to contract provisions and to ensure appropriate reimbursement for services provided. This includes tracking case statistics (e.g. direct contacts, service coordination, family training/counseling, special instruction, transportation, etc.), and accounting for at least 75% of time.
  • Complete all client-related and program-related paperwork according to established timelines. Thoroughly explain policies and procedures as it relates to services, programs, and funding sources.
  • Monitors attendance for children whose absences exceed three (3) consecutive days and notifies Admissions Supervisor for follow-up and contact with parent.
  • Responsible for keeping a cumulative record of the “Best Interest of the Child” days taken up to the limit of 10 days. Oversees effort for seeing that this is done, recorded and that all days that exceed the 10-day limit are marked as unexcused on the attendance records (where applicable)
  • Responsible for monitoring non-compliant parents regarding enforcement of late Policy, unexcused absences, excessive absenteeism, absences requiring a doctor’s note, failure to sign absence record, delinquent fees, delinquent immunizations, and failure to comply with other agency procedures or requirements by issuing a Notice of Action.
INDEPENDENCE OF ACTION: This position exercises a high level of independent responsibility in performing job tasks and is responsible for using initiative and judgment to determine how to proceed with assignments and to determine the methods best suited to each task. The position is supervised by the Admissions Supervisor, who provides direction on operational issues and are available to provide technical assistance with problems that involve other organizations. Work is performed according to Policies and Procedures established by CDE, GGRC, DEC and Felton Institute.

Minimum Qualifications:
  • Bilingual Spanish/English (REQUIRED)
  • 2-3 years related experience in social service eligibility and application processing
  • 2 years experience in data entry
  • Strong oral and written communication skills
  • Professional appearance and demeanor, with professional ethics of confidentiality, ability to exercise good judgment and discretion
  • Attention to detail, ability to manage a wide variety of tasks and meet deadlines, and reliability/dependability )
  • Must have knowledge of title V, Title 22 and Title XVII and the ability to learn and implement contract requirements & regulations.
  • Knowledge of Microsoft Word, Excel, Outlook, and other related software programs.
  • Good verbal and written communication skills in English
  • Reliability in completion of duties which include meeting deadlines
  • Must pass a Department of Justice fingerprint clearance and a Child Abuse Index clearance
  • Must be able to pass physical exam, provide negative TB test result as required by Title 22 regulations
  • Knowledge of low-income and homeless issues for children and families, preferred
  • Knowledge and experience with children with special needs and their families—preferred
