The Community Housing Coordinator is primarily responsible for coordinating support services to participants in the housing program. The main duties for this position include: conducting assessments, providing resource supports, service planning and monitoring supports for the participants.

Essential Duties & Responsibilities:

  • Accept referrals thought the Alleghenylink, review applications and interview potential clients for program eligibility and admission.
  • Secure eligibility data collection documentation requirements from clients.
  • Inspect identified apartments to meet county requirements prior to admission and during involvement in the housing program.
  • Submission of client rental vouchers for reimbursement.
  • Make client referrals for job training, education, supportive services, and housing options.
  • Completion of client service plan and progress notes.
  • Advocacy on behalf of clients to access other services.
  • Participate in county audits and ongoing communications with contract monitor.
  • Complete required reports to the county funders or any requested or assigned reports.
  • Enter and track all outcome information.

Working Conditions:

  • Work is completed within the office or out in the community at participant’s home or other community and provider agencies.
  • Must be able to travel to the various office locations.
  • Ability to lift and carry light boxes of files and supplies (less than 10 pounds) on occasion.

Requirements

  • Bachelor’s degree in social work or a Social Service-related field preferred; applicable experience with housing programs will be considered in lieu of a degree.
  • 2 years of experience working with the homeless population is preferred.
  • A valid Driver’s License and access to a vehicle.
