Pathways Care Coordinator
Job Description
TYPE: FULL-TIME (37.5 HOURS)
LOCATION: FIELD-BASED
SUMMARY
Responsible for facilitating the coordination of care for clients and their families identified for High-Fidelity Wraparound Services (Tier 1) OR Intensive Care Coordination Services (Tier 2) and meeting deliverables of the Pathways to Success Program through a System of Care lens.
ESSENTIAL DUTIES AND RESPONSIBILTIES

Clinical
  • Responsible for supporting an active caseload of 10 OR 25 families and maintaining weekly contacts via various contact methods including in person meetings as assigned per tier level. It is anticipated that actual service delivery to the family will include multiple contacts per week to provide the holistic support necessary.
  • Responsible for documentation according to Medicaid Rule 140, 141 and 132 and as outlined in the Pathways to Success Program. All documentation must be in the client file/Electronic Health Record.
  • Responsible for knowing and providing services in accordance with High-Fidelity Wraparound Model as well as CASSP and System of Care Principals.
  • Responsible for providing services in a way that is most accessible to the family.
  • Responsible for following the Pathways to Success Handbook.
  • Participate in mobile crisis response events for assigned families when possible and minimally coordinate with mobile crisis responders as expeditiously as possible, but no later than 48 hours after a crisis event.
  • Responsible for networking with collaterals on behalf of families assigned, including: referral and linkage, discharge planning with in-patient psychiatric facilities, and the advocacy of a client’s rights within the education, legal, child welfare, mental health systems, and others as needed.
  • Responsible for collaborating with community partners regarding Wraparound plans and for cohesive treatment goals/outcomes.
  • Responsible for convening and working with child & family teams where family voice and choice is practiced and celebrated.
Supervision
  • Responsible for discussing cases with supervisor in clinical supervision on a weekly basis.
  • Responsible for discussing the Wraparound model and Pathways to Success program expectations in supervision.
  • Responsible for reporting to supervisor regularly about problems that arise, need for special services and resource information.
  • Responsible for meeting with collaterals monthly to get feedback regarding the program services.
  • Responsible for consulting with Wraparound Team regarding specific client issues that include gathering assessment information and assistance in treatment planning for the client.
Administrative
  • Responsible for completion of all required documentation and clinical paperwork
  • Responsible for completion of all agency administrative paperwork
  • Responsible for attending clinical meetings on monthly basis
  • Responsible for serving on agency program committees when assigned.
  • Responsible for participation in Performance Quality Improvement Efforts
  • Responsible for compliance with Council on Accreditation Standards and Best Practices in serving clients
  • Responsible for cooperating with system evaluation.
Community Education/Public Relations
  • Responsible for meeting with collaterals monthly to get feedback regarding the program services.
  • Attend Community Engagement Team meetings as requested.
  • Attend Parent Council/Junior Board as requested.
  • Responsible for educating the community regarding the Pathways to Success Program.
  • Responsible for assisting in developing materials to be used in marketing and public relation events.
  • Responsible for facilitating positive networking relationships with community service providers and/or networks.
  • Responsible for acting as a resource to community partners about mental health and mental health services.
Training
  • Responsible for fulfilling agency requirement of training and documenting all trainings attended.
  • Responsible for attending all mandatory agency in-service trainings.
  • Responsible for attending any Healthcare and Family Services trainings or through their partner, PATH.
  • Responsible for attending and partaking in Wraparound trainings and model fidelity monitoring.
Special Assignments/Projects
  • Participate in projects as assigned by supervisor
  • Perform incidental business related travel as needed
  • Perform job responsibilities with a positive attitude
  • Responsible for promoting collegiality at the workplace by working collaboratively with others

SUPERVISORY RESPONSIBILITY
This role has no supervisory responsibilities.
PHYSICAL AND ENVIRONMENT CONSIDERATIONS
  • The incumbent will be sitting in car for travel for several minutes every day. The incumbent is frequently expected to walk, sit, stand, bend and drive. The incumbent may also need to climb stairs on occasion. Must be able to lift up to 35 lbs. on occasion
  • The work environment is mostly field based and will have varying temperatures and noise level. The incumbent can expect to work in client’s homes, schools, public libraries, community agencies, and some in office time.
  • The physical demands and work environment described here are representative of those that should be met by an employee to perform the essential functions of this job successfully.
POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time position, 37.5 hours per week.
TRAVEL
This position requires extensive travel through the assigned county(ies) including Kane, Kendall, DeKalb and occasionally outside areas.
REQUIREMENTS
Education And Experience
Bachelor’s degree in Psychology, Social Work or related field is required. Must be able to work two evenings a week. The ability to work with people from every diverse background and show cultural competency is a must. Experience working in community counseling related services and working with children and adolescents with mental health needs is desired. Bilingual in Spanish preferred.
