Client Support Coordinator

Part-Time 23 hours

$17.25 per hour

$18.25 per hour once med certification is complete plus 16 hours of PTO

Monday 10:00am-2:00pm

Tuesday 10:00am-2:00pm

Wednesday 10:00am-2:00pm

Thursday 7:00am-2:00pm

Friday 10:00am-2:00pm

**Job Summary:**
The Direct Support Coordinator will provide habilitative assistance and care to clients living in various Agency programs, including community and supported living arrangements, family support services program, and day/vocational program. Responsibilities include direct daily care, teaching, supervising clients, and maintaining accurate record-keeping.
**Responsibilities:**
- Directly supervise clients, ensuring knowledge of their status and location at all times.
- Assist in developing and implementing Individualized Plans of Service or Individual Educational Plans for clients.
- Teach, supervise, and assist clients in adult daily living activities.
- Monitor client progress on goals and collaborate with the team.
- Protect client rights and report incidents of abuse, neglect, and mistreatment.
- Maintain accurate record-keeping and implement therapy recommendations.
- Develop recreation activities for clients and collaborate with relevant parties.
- Ensure cleanliness and organization of program sites and transportation vehicles.
- Administer medications, provide routine health care, and first aid.
- Transport clients to designated activities and programs.
- Participate in emergency drills and maintain medication certification.
- Perform other duties as assigned by the Program Director or Assistant.
**Qualifications:**
- Completion of Medication Certification Program, PMT, CPR, and First Aid training.
- Valid driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Minimum two years of experience working with individuals with developmental and/or behavioral challenges preferred.

**Physical Demands:**
- Effective verbal communication.
- Ability to stand, walk, sit, handle objects, and lift up to 75 pounds.
- Good vision and basic mathematical skills.
**Language Skills:**
- Proficiency in reading and writing English.
**Reasoning Ability:**
- Ability to carry out instructions, problem-solve, and handle variables.
**Work Environment:**
Moderate noise level, potential travel by automobile, and exposure to changing weather conditions.
