**Job Summary:**
As a Direct Support Professional (DSP) working 20 hours per week in Brooks Hill, Wolcott, CT, you will be responsible for providing habilitative assistance and care to clients in various Agency programs, including community and supported living arrangements, family support services program, and day/vocational program. Your role will involve direct daily care, teaching, and supervising of clients, as well as maintaining accurate record-keeping and other duties as well.
**Responsibilities:**
- Supervise clients, keeping track of their status and whereabouts at all times.
- Assist in developing and implementing individual client plans.
- Teach, supervise, and assist clients in adult daily living activities such as hygiene, personal care, and community participation.
- Monitor client progress on established goals and report any incidents of abuse or neglect.
- Complete incident reports and medication error reports promptly.
- Implement behavioral plans and recommended therapy programs.
- Plan and implement recreation and community activities for clients.
- Collaborate with other staff, families, and agencies to provide comprehensive care.
- Maintain accurate and timely records of services provided and client progress.
- Attend staff meetings, training sessions, and required certifications.
**Qualifications:**
- Successful completion of Medication Certification Program.
- Completion of PMT, CPR, and First Aid training.
- Possession of a valid CT driver's license with a satisfactory driving record.
**Requirements:**
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals with developmental or behavioral challenges, or a comparable combination of education and experience.
**Physical Demands:**
- Ability to lift and move up to 75 pounds, exerting over 100 lbs. if needed for physical restraint (training provided).
- Required vision abilities include close vision and the ability to adjust focus.
- Regular standing, walking, and use of hands and arms.
**Language Skills:**
- Proficiency in reading, writing, and interpreting English documents.
- Ability to write reports and correspondence in English.
**Mathematical Skills:**
- Basic arithmetic skills, including addition, subtraction, multiplication, and division.
**Reasoning Ability:**
- Capability to apply common sense and follow instructions accurately.
- Problem-solving skills with multiple variables.
