The original founders of the organization were Fairbanks Alaska community members with a diverse range of professional backgrounds committed to helping children with a wide spectrum of behavioral health issues and mental illness. Today, FCSA is a large diverse organization with over 100 employees providing services in the Fairbanks area, Mat-Su Valley, and surrounding communities. FCSA provides services ranging from outpatient services, residential treatment care, educational support, and daytime child-care.

About the Job

Family Centered Services of Alaska (FCSA) is searching for a Case Manager to provide services in their community based services programs. The Case Manager is responsible for coordinating necessary services for FCSA clients, including arranging for after-care services at discharge, and oversight of the direct service delivery of client milieu. The case manager is expected to act as a liaison between the client, family, and community agencies. This position requires the use of your personal vehicle and vehicle insurance.

Roles & Responsibilities

1. Facilitate family and inter-agency collaboration.

2. Advocate for the rights and needs of the youth and act as a community liaison who will represent FCSA to the professional and social community.

3. Prepare client treatment plan.

4. Monitor preparedness for client discharge.

5. Monitor behavior and progress in relation to the treatment plan for each FCSA client.

6. Identify and assist in remediation of “gaps” in the plan of care for each youth, create appropriate services when critical need cannot be met more traditionally, and coordinate any alternate treatment placement arrangements for the client, as needed.

7. Review and process all Treatment Related Occurrence Forms (TROFs).

8. Contact parents/guardians regularly to discuss major issues/progress.

9. Coordinate travel arrangements for in and outgoing clients/families.

10. Coordinate on/and off site visits, supervised phone calls.

11. Schedule treatment team meetings in accordance with Quality Assurance timelines and standards.

12. Ensure all client documentation is entered into the Electronic Medical Records System in accordance with FCSA Quality Assurance standards and timelines.

13. On-Call–Rotation as scheduled.

Education & Qualifications

1. Bachelor's degree or equivalent work experience in psychology, social work, justice, guidance and counseling, sociology, education, nursing, or other human service field preferred.

2. High school diploma or GED, and two years working with adolescents in a mental health setting required.

3. Ability to communicate in a professional manner with appropriate stakeholders (Treatment Team Members, outside agencies, program director).

4. Effective office skills including; time management, computer proficiency, strong verbal and written skills, and ability to complete required documentation in a timely/accurate manner.

5. Physical ability to employ verbal and physical aggression control techniques after being certified.

6. Knowledge of State of Alaska childcare regulations and juvenile justice laws.

7. Freedom from previous criminal convictions that may adversely affect the employee's potential performance contingent on a fingerprint and/or records check.

8. Current Alaska driver's license, driving record and vehicle insurance..

This position is a driving position. Applicants will be required to submit a driving record if selected for the position.

All applicants must be at least 21 years of age due to State of AK regulations and must pass a State of AK Background Check.

Job Type: Full-time

Pay: $20.75 - $22.01 per hour
