As we continue to grow and expand to cover the Lakeshore, Faith Hospice has some unique opportunities to consider for a Licensed Master's prepared Social Worker (LLMSW or LMSW) who feels the call to serve those patients at end of life and desires the opportunity to work as an essential part of an interdisciplinary health care team who also share this very rare and special gift.

At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides, and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling.

What Opportunities are available?

• MSW Community: manage a caseload of hospice patients, Monday - Friday schedule
• RMSW Admissions: informational and sign on visits when appropriate

Education and Experience

• MSW licensure in Michigan (LLMSW or LMSW)
• At least 1 years of medical social work experience
• Hospice and/or hospital experience preferred
• Passion for hospice social work
