RN Nurse Coordinator, 2nd shift – Trillium Woods

We are seeking a compassionate RN who has the very special gift of hospice nursing to assist our Residence Manager as the RN Nurse Coordinator at Trillium Woods, our beautiful 20-bed inpatient hospice residence.

What will you do?

• Work a schedule that complements the Residence Manager: Mon-Fri, 2pm – 10p
• Responsible to assist in the supervision of the daily operations at our 20-bed hospice residence, being present on the floor and assisting the care team when needed
• Participate in the Hospice interdisciplinary group meetings
• Case manage doing weekly assessments, recertifications, and care conferences
• Assist with admissions when needed
• Backfill when the Residence Manager is out of the building
• Assist Residence Manager with hiring, orienting, and ongoing coaching of the care team
• Assist with covering for open shifts when needed
• Participate in the manager on-call rotation with the other hospice managers

What is needed to be successful in this role?

• This opportunity requires licensure as an RN in the State of Michigan
• Minimum of 1 year of nursing supervision in a skilled nursing or med/surg setting
• Prior management and hospice nursing experience is highly preferred
• Strong computer skills to chart in the EMR are essential.
