Job Description: Physical Therapist - Glasgow Health and Rehab

Sign on Bonus: $10,000

Position Summary: As a Physical Therapist in our long-term care facility located in Glasgow, WV, you will be responsible for providing high-quality rehabilitative care to our residents to enhance their mobility, strength, and overall functional independence. Your role will involve conducting assessments, developing individualized treatment plans, and implementing evidence-based interventions to optimize residents' physical function and quality of life.

Key Responsibilities:

  • Conduct comprehensive evaluations of residents' physical function, mobility, balance, and strength to identify impairments, functional limitations, and rehabilitation needs.
  • Develop personalized treatment plans based on assessment findings, resident goals, and clinical reasoning, utilizing a variety of therapeutic modalities and techniques.
  • Implement therapeutic exercises, manual therapy, gait training, balance activities, and other interventions to improve residents' range of motion, muscle strength, endurance, and coordination.
  • Provide hands-on assistance and guidance to residents during therapeutic activities and functional tasks to promote safety, independence, and optimal recovery.
  • Monitor residents' progress, adjust treatment plans as needed, and document treatment sessions accurately and in a timely manner, adhering to regulatory standards and facility policies.
  • Collaborate with interdisciplinary team members, including occupational therapists, speech therapists, nurses, social workers, and physicians, to ensure coordinated and holistic care for residents.
  • Educate residents, families, and caregivers on therapeutic exercises, home exercise programs, fall prevention strategies, and adaptive equipment to support ongoing rehabilitation and maintenance of gains.
  • Participate in facility meetings, case conferences, and quality improvement initiatives to enhance clinical outcomes, promote best practices, and contribute to the overall excellence of resident care.

Qualifications:

  • Master's or Doctorate degree in Physical Therapy from an accredited program.
  • Current state licensure or eligibility for licensure as a Physical Therapist in West Virginia.
  • CPR certification and compliance with continuing education requirements.
  • Previous experience in long-term care, geriatric rehabilitation, or skilled nursing preferred.
  • Strong clinical skills, critical thinking abilities, and a compassionate approach to patient care.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with residents, families, and interdisciplinary team members.
