Job Summary: We are seeking a skilled and compassionate Visiting Nurse to provide high-quality healthcare services to our patients in their homes. As a Visiting Nurse, you will play a vital role in delivering personalized care and promoting the overall well-being of our patients. Responsibilities: - Perform comprehensive assessments of patients' health status - Administer medications and treatments as prescribed by physicians - Monitor and record vital signs, including blood pressure, heart rate, and temperature - Provide health coaching and education to patients and their families - Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans - Assist with discharge planning and coordination of post-hospital care - Conduct diagnostic evaluations, such as wound assessments and laboratory specimen collection - Provide specialized care for patients with memory impairments or behavioral health needs - Collaborate with ophthalmologists for eye care assessments and treatments - Utilize Epic electronic medical records system for documentation and communication - Engage in case management activities to ensure continuity of care Requirements: - Valid nursing license in the state of [LOCATION] - Proven experience as a Visiting Nurse or in a similar role - Proficiency in catheterization procedures - Strong knowledge of vital signs monitoring and interpretation - Familiarity with health coaching techniques - Experience in discharge planning and post-hospital care coordination - Ability to perform diagnostic evaluations, such as wound assessments and specimen collection - Knowledge of memory care principles and behavioral health interventions

Pay: $90.00 - $100.00 per hour
