What is PACE?

PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age requiring nursing home level of care which aims to keep participants living at home in the community. PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.

The Quality Improvement Coordinator develops, implements, and monitors the PACE Quality Improvement (QI) plan. The QI Coordinator ensures that data is collected, examined, and shared with all appropriate stakeholders. The QI Coordinator is responsible for compliance with state and federal regulations and standards of practice, ensuring compliance by facilitating planned and ad hoc audits; tracking and trending results; and facilitating action plans as necessary based on audit findings. The Quality Improvement Coordinator reports to the Director of Quality and works closely with the PACE Medical Director all staff in the PACE Center. 

Primary Duties and Responsibilities:

  • Develops and guides the implementation of the PACE Quality program. 
  • Ensures staff integration into the QI process through Continuous QI education and developing opportunities for input. 
  • Performs analysis of QI activities. 
  • Oversees PACE Participant satisfaction. 
  • Participates in the PACE Participant Advisory Committee. 
  • Oversees federal audit preparation with CMS along with Esperanza Director of Risk and Compliance and Vice President of PACE. Maintains continual survey readiness for the PACE Center.
  • May function as the Infection Prevention and Control Coordinator. 
  • Stays current with and ensures ongoing compliance with PACE State and Federal regulations and quality reporting requirements. 
  • Works with PACE Systems Analyst to create necessary reports for monitoring performance with quality reporting requirements.
  • Submits required reports to CMS and HFS before all deadlines as defined by regulatory requirements.
  • Tracks and trends quality metrics; creates action plans when improvement is needed.
  • Educates PACE team on changes to PACE quality reporting requirements.
  • Communicates with supervisor and as needed, to report all areas of concern/issues/incidents. 
  • Works collaboratively with the PACE Medical Director on quality improvement projects. 

Requirements

  • Bachelor’s and/or master’s degree in health discipline (e.g. MPH, MHA, or clinical professional degree) with two or more years’ experience in healthcare required; may substitute additional relevant experience for degree requirement.
  • Minimum of one year of demonstrated experience in QI activities in a health-related work environment. 
  • Experience in a heavily regulated environment (TJC, HRSA, DNV, etc.) preferred.
  •  CPHQ preferred.
