Compensation (Based on experience and qualifications) : $21- $26/hour
Location: Hybrid / 1940 S Western Ave. Chicago, IL 60608
Schedule: Monday - Friday, may include Saturday hours
Esperanza Health Centers, Federally Qualified Health Center, is seeking an organized, strategic and patient service driven leader to join their Referral Coordination team. The Lead Referral Coordinator serves as a subject expert on referrals from specialties offered at Esperanza. The responsibilities will be also encompass hands on work to assist with timely processing of all referrals.
Primary Duties and Responsibilities:
- Implement a solutions-focused approach to problem solving, building and maintaining buy-in, to contribute to a healthy work culture.
- Use best practices to oversee the development, implementation, and ongoing care management of Esperanza patients.
- Perform quality assurance reviews on existing referrals through reviews, chart audits, and ongoing training.
- Educate the referral coordination team to ensure patient safety and compliance with Esperanza, State, Federal, and health plan guidelines.
- Utilize all healthcare technologies to evaluate department workflows, measure goal progress, identify areas for improvement, and ensure a positive impact to all patients
- Collaborate with other clinical teams to create strategies that maximize operational efficiencies, improve clinical outcomes, and great patient experience.
- Conduct regular audits for position development, which includes: patient chart reviews, troubleshooting complex patient cases, establishing and maintaining external partnerships

Requirements
- Two or more years of experience in a clinical or hospital setting, required
- Knowledge of Medicare, Medicaid, and Marketplace benefit coverage
- Experience with Electronic Health Records
- Excellent verbal and written communications skills
- Bilingual Spanish/English, required
