Now Hiring: Memory Care Coordinator
Responsibilities Memory Care Coordinator:
- Conduct comprehensive assessments of residents' cognitive and functional abilities.
- Develop and implement individualized care plans based on assessment findings.
- Provide training and support to staff members on memory care best practices.
- Design and oversee engaging and therapeutic memory care programs and activities.
- Collaborate with healthcare professionals for comprehensive resident care.
- Communicate regularly with residents' families, offering support and updates on care plans.
- Manage medication administration and compliance with protocols.
- Handle crisis situations effectively, implementing appropriate interventions.
Qualifications of Memory Care Coordinator:
- Bachelor's or Master's degree in Nursing, Social Work, Gerontology, or a related field.
- Previous experience in a memory care or dementia care role.
- Certification as a Certified Dementia Practitioner (CDP) or equivalent is preferred.
- Strong interpersonal and communication skills.
- Compassion, patience, and a commitment to providing exceptional care.
