Job Description

Position Objective: Responsible for delivering nursing care to people supported by utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. The LPN will effectively interact with people supported, significant others, residential staff, CDS staff, and health team members while maintaining all standards of professional nursing. Responsible for rendering nursing care in assigned areas in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse.

Essential Duties/Responsibilities

  1. To support the philosophy through the agency’s mission, vision, and values.
  2. Perform focused nursing assessments on persons served for acute medical problems as directed by the RN(s).
  3. Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic, and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
  4. Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse.
  5. Administer medication accurately, observing the individual's response, as evidenced by documentation in the medical record and lack of negative outcomes.
  6. Maintain and coordinate communication among person supported, guardian, staff and physician as evidenced within medical record, client, and healthcare team feedback.
  7. Assist in assuring that effective communication exists with person supported as reflected in the plan of care.
  8. Give punctual change of shift report consisting of pertinent and relevant information for persons supported.
  9. Work collaboratively with other members of the healthcare team in coordination of the person's care as evidenced by other department feedback.
  10. Use available written materials specific to a person’s condition or disease.
  11. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration or discussion.
  12. Assist persons supported, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  13. Answer and refer telephone calls or other inquiries to ensure accurate and timely communications are facilitated.
  14. Communicate with co-workers, management, persons supported, families, and others in a courteous and professional manner.

  1. Administer medications according to policy and procedure.
  2. Observe and document a person’s responses to medications/treatments.
  3. Demonstrate the ability and appropriate technical skills when administering treatments and procedures in accordance with the physician's orders, policy, and procedure.
  4. Demonstrate the necessary skills and knowledge to provide care for people according to policy, procedure, regulations, and site-specific competencies.
  5. Demonstrate ability to handle emergencies in a prompt, precise, and professional manner.
  6. Provide personal care to ensure comfort and well-being while acknowledging physiological and psychological needs.
  7. Perform indirect care, using established procedures, policies, and standards as evidenced by feedback and observation.
  8. Communicate pertinent information by phone, fax, email, and electronic programs to the appropriate people in a timely manner including people supported, guardians, and needed EP!C team members.
  9. Demonstrate accurate and timely documentation (written and electronic) according to guidelines.
  10. Maintain a clean, neat, and safe environment for people and staff.
  11. Ensure that supplies and equipment necessary for care are stored in an organized and efficient manner.
  12. Demonstrate correct and safe techniques in the use of equipment according to specific product information.
  13. Ensure that medications/treatments are kept in a secure and safe location.
  14. Follow appropriate procedures for obtaining and returning or cleaning/disposing of equipment and supplies.
  15. Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.
  16. Adhere to administrative system/nursing service policies.
  17. Remain conscientious regarding personal hygiene and comply with regulations.
  18. Comply with specific agency and DHS policies and procedures concerning safety.
  19. 100% attendance to mandatory skill updates as required.
  20. Use the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the person's care, as evidenced by observation and reporting by the person, family, and staff members.
  21. Assess the person to collect and document their current health status and medical history. Make accurate and ongoing assessment of their status responding and documenting appropriately.
  22. Set priorities for nursing action in a logical sequence according to person’s needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards.
  23. Perform ongoing assessments and revise the plan of care based on new data and the person’s condition.
  24. Accurately document care to include but not limited to nursing intervention, the person’s response to care provided, needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate teaching with those needed and ensuring needed instructions is noted in the medical record. Collaborate with other nurses and health team members to ensure the person's well-being as evidenced by feedback from the person, their family, and staff.
  25. Perform direct care, using established procedures, policies, and standards as evidenced by feedback and observation.

Marginal Responsibilities

  1. Perform other duties as assigned.
  2. As a mandated reporter, ensure that all allegations of abuse and neglect are reported as required.

Education/Experience Requirements

  1. Graduate of an accredited school of nursing.
  2. Valid registration as a Licensed Practical Nurse.
  3. Completion of relevant state Board of Nursing approved.
  4. Current AHA Healthcare Provider Training (BCLS).
  5. 1 year experience in medical nursing.
  6. An equivalent combination of education and experience considered.

Physical Requirements

  3. Other physical activities related to healthcare and office environment.
  4. Operates general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer.
  5. Ability to lift 50 lbs.
  6. Operates general nursing equipment.

Work Environment

  1. Work may occur in the following settings: Home, Office, Classroom, Site, Group Homes, or Community.
  2. Will be required to drive a motor vehicle (EP!C’s and employee’s) as part of his/her employment responsibilities with EP!C. A valid Illinois Driver’s License and insurance is required and must be maintained at all times while employed by EP!C.
