Elohim Group LLC is Hiring a Rehabilitation Specialist LMSW, LGPC, CPRP, CFRP - Onsite Near Baltimore, MD

Our client is a growing mental health organization in Maryland and is devoted to helping individuals refine their purpose & restore holistic peace, balance, and wellness.

Its Psychiatric Rehabilitation Program (PRP) is looking for Rehab Specialists to add to our team of passionate, caring, and reliable Professionals. As a Holistic organization, we meet our clients where they are by offering comprehensive services to support their treatment: Individual, Family, and Group Therapies, Psychiatric evaluations, Medication Management, Psychiatric Rehabilitation, and Addiction Services.

REHABILITATION SPECIALIST ROLE SUMMARY

  • Provide oversight for the daily program of rehabilitation services;
  • Provide training and supervision to direct care staff during their initial orientation and throughout their tour of service;
  • Educate direct care staff on how to provide rehabilitation services that correlate to the goals identified in the Individual Rehabilitation Plan and how to appropriately document such services;
  • Review documentation of services, assessments, IRPs, summaries, and reports;
  • Responsible for the documentation and maintenance of client’s clinical records
  • Responsible for initiating the Individual Rehabilitation Plan (IRP) and/or Individual Treatment Plans (ITP)
  • Works in collaboration with the program director and the direct service staff to review IRPs
  • Conducts Individual Rehabilitation Plan (IRP) at minimum, every three (3) months for children, and every six (6) months for adults
  • Works in collaboration with the program director and the Administration’s ASO to prepare discharge plans
  • Ensures that the program provides rehabilitation activities directed toward the development or restoration of skills, including self-care and social skills
  • Ensures that all services are age-appropriate, including group and self-directed leisure activities, both on-site and off-site
  • Co-Facilitates Multidisciplinary Treatment Team meetings
  • Provides assessment, counseling, community support, crisis/emergency, and social work treatment interventions in relation to psychiatric diagnosis
  • Provides referrals to other public and private community agencies upon discharge of client for mental health support
  • Available for twenty-four (24) hour on-call responsibility and flexibility to be present during crisis situations
  • Develop and assure rehabilitation services that reflect and meet the needs of the individuals served by the program;
  • Ensure the development of client-specific, culturally appropriate intervention strategies including goal setting, planning, and decision-making;
  • Perform crisis intervention and complete Incident reports, as necessary;
  • Participate in clinical training, treatment team meetings, and staff meetings;
  • All other duties necessary to fulfill program operations

WHAT WE NEED FROM YOU 

  • Licensed mental health professional (LGSW, LGPC, LCPC, or LCSW-C); or
  • Rehabilitation counselor certified by the Commission on Rehabilitation Counselor Certification; or
  • Bachelor’s prepared certified psychiatric rehabilitation practitioner certified by the U.S. Psychiatric Rehabilitation Association.
  • At least 2 years of direct care experience working with emotionally disturbed youth
  • At least 2 years of supervisory experience
