WHAT YOU'LL DO:

The Health Programs Specialist provides individual and group education on chronic disease management, adhering to a predetermined curriculum. This includes coordinating and collaborating with clinical and non-clinical staff to ensure patients are provided with the best possible care and counseling. This individual will also be expected to consider socioeconomic, social, religious, cultural, and other personal values. The Health Programs Specialist will be primarily involved in the Healthy Seniors programs, but will have opportunities to expand into other areas of health programs. The programmatic responsibilities of this position may change depending on departmental needs.

Compensation Dependant Upon Experience

$22.00 - $28.60 an hr

Requirements

WHAT ARE WE LOOKING FOR?

The successful candidate will be willing and able to:

  • Schedule and complete Healthy Hearts, Bone Health, and IBT for Obesity coaching sessions, adhering to the provided curriculum
  • Identify eligible patients through database reports and lead efforts for patient outreach and enrollment into the Healthy Hearts, Bone Health, and IBT for Obesity programs
  • Help members understand hypertension, dyslipidemia, osteopenia, osteoporosis, and obesity
  • Employ confidence-promoting techniques in patient communication and develop patient self-efficacy to better manage health.
  • Use behavior change techniques to help members develop healthy habits.
  • Assist/support members in safely setting and achieving goals such as losing weight, increasing physical activity levels, and making healthier food choices to lower risk for developing hypertension, dyslipidemia, and obesity.
  • Document member progress and results accurately and completely.
  • Contribute to discussions on challenges and process improvements by listening, sharing input and ideas, and enacting decisions and plans.
  • Maintain confidentiality of all regulated information in compliance with state and federal laws, including but not limited to Protected Health Information (PHI) and Personal Identifiable Information (PII).
  • Work collaboratively with Care Team to optimize communication, coordinate needed services and meet the needs of the patient
  • Serve as facilitator and notary for the Advance Directive program

The successful candidate has:

  • Bachelor’s degree in health promotion, health education, or a similar field strongly preferred.
  • 1 year(s) of relevant experience, particularly with diverse and/or economically disadvantaged populations
  • Pass the State of California notary exam within 3 months of hire
  • Professional demeanor and approach to care
  • Adaptable and flexible communication and educational style
  • Ability to complete accurate and timely documentation
  • Excellent verbal, written, and presentation skills
  • Ability to work independently (with minimal supervision)
  • Ability to be highly organized and accountable, including ability to prioritize multiple tasks
  • Knowledge of Windows, Microsoft Office, and proficiency in navigating through various cloud-based/web-based portals and applications

Additional Requirements

  • Must have a current and valid California driver’s license and the ability to provide proof of personal auto insurance on the vehicle driven during working hours.
  • If selected for an employment opportunity with Elica Health Centers, external hires must provide proof of immunizations (Hepatitis B, MMR, Varicella & Tetanus), tuberculosis clearance, and proof of COVID-19 vaccination status* prior to their scheduled start date. Physical Requirements and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
