Job Title: Nurse Care Consultant/Coordinator

Requirements:

• Bachelor's degree in nursing/RN
• Minimum of four years in the field of eldercare 
• Experience working with individuals with dementia and/or cognitive impairments
• Valid driver's license and own a car 
• Job is centered around client locations (central Fairfax County, Arlington, Alexandria)
• Ability to drive clients to occasional physician appointments and serve as their advocate
• Physical Requirements: ability to push a wheelchair, fold and place a wheelchair into car, and stand by assistance with transfers

• Ability to serve on-call after hours within a rotating schedule

Desired qualifications:

• Senior Living experience (skilled nursing, memory care, assisted living, and/or rehab) 
• Hospital experience
• Comfort working with older adults with mental illness
• Familiarity with local eldercare resources

Duties and tasks:

• Complete comprehensive assessments
• Design individualized working care plans
• Coordinate, manage, and oversee care
• Experience in home care and/or skilled nursing/assisted living communities a plus
• Accompany clients to physician appointments
• Advocate for client needs in-home or facility
• Communicate with those involved in client’s care
• Contribute knowledge base to ElderTree team
