Elder Options providing care management & home care services in the El Dorado Hills/Placerville area is looking to add a Care Manager to our team. Minimum education is B.S. degree in social work, gerontology, R.N. , LVN or other related human services field. Masters Degree a plus!
Must have ability to work with a high degree of autonomy & professionalism. Supervision under a Clinical Supervisor. Good computer & communication skills required. Ability to set your own schedule & work independently on client/family issues. We are looking to add to the Care Management team and would like to hire a professional who is seeking to work 15-20./week. Direct practice a must including making home visits, assessments, understanding of older adult issues including entitlement programs.

Duties include:
Initial & monitoring client/family home visits , individual person centered care plan development, communication with client/families. Identify unmet needs, wishes & expectations with client. . Documentation skills necessary on industry specific software. Knowledge in making referrals, understand program eligibility, and coordinate care needs with community resources.
Assigned caseload after comprehensive training will be west slope of El Dorado County/Folsom area. Caseload will be managed with a high degree of autonomy and have experience of working with clients & families in a community setting.

Non billable Admin Rate: $20.50/hour
CM Billable: $30-$32 per hour
