We are currently hiring a School Based Health Center Community Health Worker based in Worcester MA. The School Based Health Center (SBHC) Community Health Worker (CHW) works as a member of a the SBHC Burncoat multidisciplinary team including clinical and administrative staff across service lines and the Public-School staff to provide comprehensive, coordinated interventions to address patient’s social determinants of health (SDOH). The SBHC CHW promotes, facilitates, and improves access to comprehensive healthcare and social services for youth and their families. The SBHC CHW provides interventions in accordance with the Massachusetts Board of Certification of Community Health Worker core competencies as part of a patient-centered medical home.

Job Summary:

  • Conducts assessments for Social Determinants of Health (SDOH) with patients referred for CHW interventions in accordance with health center policies and protocols. Develops an intervention plan based on identified needs to provides assistance and support to patients.
  • Facilitates access to care at SBHC by providing administrative assistance support that includes patient registration and enrollment; scheduling; and preparing patients for medical visits.
  • Provides basic education, screening, brief intervention, and facilitates referrals as needed to other community-based resources in accordance with applicable health center protocols.
  • Provides health system education and advocacy to assist patients in navigating the health care system.
  • Participates in population-based projects, for example, reaching out to assure preventive care is received, follow up of chronic missed appointments, and/or adherence to chronic care guidelines.
  • Follows up with provider-identified patients to further assess and address psychosocial barriers that limit patient engagement in care

Job Requirements:

  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Depending on the nature of the position, bilingual in English and another critical language for the health center may be required.
  • Successful completion of the CHW core competency training within 1 year of employment
  • BLS Certification
  • Our health center requires all employees to have the most recent COVID-19 booster and the yearly flu vaccine.
