Position Summary:
The Community Health Worker – Medication Refill Program is a role offering support to the entire population to improve chronic disease medication adherence. The CHW-Medication Refill will work within the Medication Refill team to coordinate care, educate and support patients in a holistic approach to their health. The CHW-Medication Refill will be expected to advocate for patients, connect them back to their Primary Care Provider and other community resources or support. They will also play a crucial role in engaging patients in their medication adherence through education and facilitating medication refills.
Program Specific Expectations:
•Facilitate coordination of services for patients.
•Collaborate with other members of the medication refill and the Primary Care team including Providers (both Medical and Behavioral Health), Nurses, Clinical Pharmacists, Medical Assistants, and other Community Health Workers.
•Conduct preliminary screening and assessment of patients who need support with adherence to medication including addressing social needs impacting ability to stay on medications, transportation issues to prevent patients from getting refills, etc.
•Assesses and connects patients to their other routine health maintenance needs.
•Provides support by telephone.
•Listens empathetically, providing culturally appropriate support.
•Advocate for patients’ needs and support care coordination to meet those needs.
Program Essential Duties and Responsibilities:
•Provide support services to clients.
oConducts preliminary screening of patients to assess barriers to medication adherence including medication noncompliance, food and housing insecurities or other barriers to care.
•Help solve simple problems.
oListen attentively.
oProvides support.
oDocuments participants’ problems and the recommended suggestions.
oFollows-up on recommended changes to assess effectiveness.
•Provide referral to appropriate health and/or social services
oIdentifies when a referral is needed.
oArranges appointment.
oContacts Coordinator for assistance when needed.
oDocuments referrals in logs accurately and timely.
oComprehensive assessment of health-related social needs and appropriate referrals.
•Coordinate and integrate care
oWorks with Primary Care Services, Behavioral Health, Outside agencies.
oProvides feedback and follows-up on care.
•Participate in trainings.
oParticipate in Community Health Worker (CHW) training in order to expand their skillset to provide support to patients.
oParticipate in other trainings as required
Qualifications and Education Requirements:
One of the following combinations of education and employment experience must be met to be considered for the position:
EducationAndExperience:
GED/High School Diploma required. Coursework towards an Associate’s degree or Bachelor’s degree in human services or closely related field of study preferred.
Bilingual in English and another language relevant to FHCW population. •Lived experience with Chronic Disease Management preferred
•Strong communication and advocacy skills required.
