General ResponsibilitiesReporting directly to the Director of Early Intervention, the Speech Language Pathologist (SLP) provides speech and language services in the areas of assessment, direct therapy, training and consultation to staff and families of children age birth to three in natural environments as a primary service provider. The SLP serves as a liaison with medical and community service providers on behalf of children with disabilities and their families.Assignment/Location:This position is full time, 185 day contract and will serve our Early Intervention program. We are building an applicant pool for 2024-2025 anticipated openings. Your application will be considered as positions become available. Estimated start date will be August 2024.Essential Functions/Other FunctionsEssential Functions:
Inform teachers and other school personnel of speech, language and/or hearing services.
Provide formal and informal evaluations assessing functional capabilities of infants and toddlers on assigned caseload.
Develop and implement specific goals with measurable outcomes in the Individual Family Service Plan in conjunction with the IFSP team.
Maintain all Early Intervention documentation including EI state forms and written documentation of all evaluations, goals, speech/language treatment plans, progress notes, and communication with doctors and families, in accordance with agency formats in a professional manner and in compliance with established time frame.
Participate as a member of a multidisciplinary team.
Ensure the goals of the infant, toddler, and family are met as stated in the Individual Family Service Plan (IFSP).
Provide consultation and home programs for the families/caregivers to facilitate goals as identified in the Individual Family Service Plan (IFSP).
Other Functions:
Other duties as assigned.
Availability to consult for other disciplines on an as-needed basis.
