Edison Home Health Care is looking for a Nurse Coordinator to join our home care agency's Nursing team. The Nursing Coordinator will work primarily from our Brooklyn office and be an integral part of the department.

Responsibilities:

• Initial Report/ Start of Care pulled up daily
• Discharge Report to be run daily
• Reassessment Report to be run weekly
• Assigning RN to complete the visits
• Following up with RN on the fist day of service to remind them about the visits and confirming it was completed
• Following up on RN paperwork
• Saving all documents in patient folder
• POC to be entered in exchange
• MD Order to be send out
• HHA supervision to be send to HR
• Completes Quality Survey JOT form
• Mail POC to patient if needed
• Help RN schedule visit if needed
• Assist with authorization for visits as needed
• Have knowledge and be able to use the software’s we are currently using
• Assigning Incident visits to RN when it is received via smart sheets

Requirements:

• High school degree or equivalent required.
• Bilingual Spanish Preferred 
• Experience in healthcare/home care. - Preferred
• Knowledge of HHA Exchange. - Preferred
• Comfort with Microsoft Office especially excel and similar programs.
• Ability to communicate effectively with employees, patients, and their families
• Ability to define problems and tasks, collect data and establish facts, take action and resolve issues
