Easterseals Blake Foundation is Hiring a Community Health Nurse (Tucson) Near Tucson, AZ

Description

Because of the strong link between substance abuse and child abuse as well as Arizona Governor’s declared state of emergency around the Opioid epidemic, EBF has built collaborative partnerships between community agencies, including Healthy Families, substance abuse treatment services, and Home Nurse Visitation services to provide services to families of substance exposed newborns. Within the first 30 days the nurse will conduct a home visit to assess the child(ren)’s general health including well-being, physical condition, and postnatal mood disorders.

The EBF nurse will discuss the children’s health and development in depth with the parent(s)/caregiver(s), specifically addressing the parental substance abuse and its effects on the well-being and care of the children. In the last thirty days, the SENSE nurse will conduct a second In-Home visit with the family to identify and address differences from the first visit, including any progress, concerns or both.

Essential Duties and Responsibilities:

  • Assesses the referred child(ren)’s general health including but not limited to, well-being, physical condition, and postnatal mood disorders.
  • Assesses the children’s development using a development screening tool as approved by ADCS at each visit.
  • Ensures that the referred mother completes the Edinburgh Postnatal Depression Scale at each visit.
  • Provides hard copies of the completed developmental screenings as approved by ADCS, and the Edinburgh Postnatal Depression Scale to the manager for distribution within seven days of completion.
  • Provides infant safety resources to the family emphasizing safe sleep practices, infant vision, Fussy Baby/Helpline, feeding, developmental milestones, head shape, and other resources as needed.
  • Documents the physical environmental conditions.
  • Verbally shares concerns during the assessment with parent(s)/caregiver(s) and SENSE team members.
  • Completes the Health Consultant Nursing Assessment (HCNA) at each visit and email to the manager.
  • Between first and second visit, identify and address differences from the first HCNA, including any progress or concerns.
  • Submits the Health Consultant Nursing Assessment (HCNA) to the ADCS Specialist once completed.
  • Attends quarterly collaboration meetings facilitated by ADCS.
  • Demonstrates sensitivity to linguistic, cultural, social, economic, individual and role differences among persons and families served.
  • Explains the benefits and responsibilities of the program to families in a manner which is sensitive and responsive to individual family needs.
  • Works in collaboration with the SENSE team and families to assess the families’ educational, social, health and economic needs; develops an intervention plan and assists them in attaining their established goals.
  • Identifies and disseminates relevant resource and service information to families and co-workers as appropriate.
  • Any other duties as assigned.
  • Functions as an effective team member with co-workers and the community to build productive, collaborative relationships and to achieve program goals and objectives.
  • Keeps appointments, is punctual, and utilizes time effectively.
  • Shares pertinent information with team members regarding work progress, successes, barriers, concerns that may have an impact on program quality.
  • Communicates in a courteous, positive and respectful manner.
  • Interacts with co-workers and the community to build productive, collaborative relationships to support the achievement of service goals.
  • Understands and maintains private and sensitive information within the confines of need-to-know parameters, using HIPAA guidelines and rules.
  • Submits accurate and complete screenings, assessments, interventions and other written information as required.
  • Proofs and revises work for correct grammar, spelling and format.
  • Oversees the implementation of and monitors the delivery and quality of service.
  • Promotes family sufficiency related to health issues, and measures desired participant and family outcomes.
  • Provides verbal and written information to families to promote/support an increased understanding of growth and development norms.
  • Provides verbal and written information to families to promote/support increased knowledge of health promotion practices, including prevention.
  • Provides verbal and written information about the importance of parent-child interaction that is appropriate to age and development.
  • Identifies children with physical and/or developmental problems and provides appropriate referrals for evaluation and intervention.
  • Coordinates and conducts fiscal activities as assigned and assures the assets of the program are utilized in a safe and efficient manner.

Requirements

Education –

  • Arizona Licensed Registered Nurse, BSN or MS preferred

Experience –

  • Pediatric birth to five care; Newborn Intensive Care Unit
  • Completes statewide SENSE training upon hire, per ADCS contract requirements
  • Bilingual in Spanish and English preferred, not required.

Regulatory –

  • Minimum 21 years of age.
  • Drug test prior to hire then annually per ADCS contract requirements.
  • Clean criminal background check and the ability to obtain and maintain a level one clearance card.
  • Valid, unrestricted Arizona driver’s license and the willingness to use personal vehicle in the course of employment. Must meet minimum requirements of agency insurance carrier.
  • Must possess a valid AZ DPS Level 1 fingerprint clearance card.

Physical requirements:

  • Must be able to utilize Personal Protective Equipment (PPE), such as face mask, gown, goggles, and gloves, for any amount of time necessary to provide a physical barrier to protect the employee and others.
  • Must be able to remain in a stationary position.
  • Must be able to operate a computer, phone, and other office equipment.
  • Must be able to stoop, squat, reach, stretch, ascend, and descend stairs.
  • May work in an office setting.

  • Must be able to remain calm, non-defensive with a supportive attitude during stressful, potential crisis, and crisis situations.
  • Must be able to work in an extremely stressful environment where there are considerable mental and emotional demands.
  • Must be able to manage high volumes and unpredictability of work.
  • Must be able to remain flexible and reprioritize according to the changing demands of the day.
  • Must be able to manage stress in a fast-paced, unpredictable, intense setting.
  • Must be able to adapt to diverse cultural environments and communicate in a culturally inclusive manner.
  • Must be able to represent Easterseals Blake Foundation in a professional manner in all situations.
