DuPagePads is Hiring a Health Case Manager Near Downers Grove, IL

Position: Health Case Manager

Summary: Medical issues are significant factors in the onset, experience, and resolution of homelessness. This position will serve as the primary case manager for individuals and families with health challenges to achieve their wellness and housing goals. Through the relationship with clients, this position will provide direct care services that will guide, educate, and motivate individuals experiencing homelessness, and will serve as the medical benefits and resources expert for individuals served in the program. This position will ensure individuals have medical coverage, understand their coverage, and are able to successfully able to utilize their benefits.

Supervises: N/A
Responsibilities: (to be performed with or without reasonable accommodation):
  • Operates within the DuPagePads Core Values, which supports providing trauma-informed care.
  • Assists individuals with obtaining and maintaining health insurance coverage (i.e. Medicaid, Medicare, etc.). Develops systems to ensure individuals do not have gaps in coverage by addressing re-determinations, changes in eligibility, and spenddowns. Educates clients on available options and benefit-related concepts. (i.e. co-pays, deductibles, etc.)
  • Works collaboratively with individuals to establish a primary care physician and relevant specialists.
  • Resolves barriers to access. (i.e. obtains referrals approvals, files appeals, manages other documentation to ensure continuity of care, etc.)
  • Maintains confidentiality and adheres to client privacy laws; demonstrates cultural sensitivity.
  • Develops relationships with healthcare providers and insurers, serving as advocate to clients.
  • Remains knowledgeable about changes in healthcare systems (i.e. health homes, managed care, parity) that may impact the individuals served.
  • Collaborates with clients to assess and prioritize healthcare and housing needs while developing an understanding of individual strengths and barriers.
  • Accompanies clients to healthcare appointments to provide support, advocacy, assistance with communication, and care coordination.
  • Provides individual support and health education to reach healthcare goals. Utilizes psychoeducational and other interventions (i.e. Motivational Interviewing, Harm Reduction, etc.) to teach self-care, goal achievement, lifestyle, nutrition, and disease self-management.
  • Plans, coordinates, or facilitates educational workshops, activities, or group outings.
  • Provides or coordinates transportation of clients to and from appointments or related activities.
  • Provide appropriate referrals to community services.
  • Prepare and maintain accurate confidential case files, contractual compliance and paperwork, and status reports.
  • Provide individual case management to participants to establish goal plans to include long-term objectives for income and housing stability.
  • Participates in agency staff meetings and trainings as directed by supervisor.
  • Other duties as assigned by supervisor.
Knowledge, Skills and Abilities:
  • Unconditional positive regard and non-judgmental approach to services for individuals.
  • Ability to motivate individuals to action through helping clients to understand and navigate choices while maintaining a deep value in self-determination.
  • Knowledge of healthcare system, willingness to learn.
  • Knowledge of issues related to homelessness, poverty, physical health, mental health and substance use disorders.
  • Ability to engage with clients and professionals in community settings (i.e. non-DuPagePads settings such as health clinics, pharmacies, etc). Includes effective crisis intervention skills.
  • Excellent interpersonal and engagement skills.
  • Strong organizational skills and attention to detail.
  • Creative problem-solving skills.
  • Ability to plan and analyze.
  • Ability to work independently and work as part of a team.
  • Ability to maintain accurate records in compliance with applicable regulations and standards.
  • Computer literacy in Microsoft Word and Excel, database, and presentation software.
Qualifications:
  • Bachelor's degree in Social Work, Psychology, or a related field.
  • 1 year experience in case management.
  • Experience providing health-focused case management (i.e. hospital social work, healthcare navigation, health education, patient educator, discharge planning, etc.)
  • Computer skills with proficiency in Microsoft Office or similar. Familiarity with electronic recordkeeping preferred.
  • Flexible, adaptable and have the capability to work in a fast paced, professional environment.
  • May be required to become First Aid/CPR certified
Other:
Flexible hours required.
Be able to lift 10 pounds.
Provide own transportation and possess a valid Illinois Driver’s License.
Classification Status: Full-time Non-Exempt
Starting Pay Rate: $21.00/hour
