General Responsibilities

  1. Operate as a member of the Community Services team.
  2. Conduct comprehensive needs assessments and provide coordination of services to assist the identified population to gain access to and receive needed services.
  3. Provide rapid re-housing counseling, consultation, training, crisis intervention, follow up and case management services to shelter residents in need of permanent housing.
  4. Monitor ongoing client services, advocate as needed for assigned client caseload.

Essential Functions

  1. Help newly homeless persons and those at imminent risk of homelessness identify and implement strategies that will prevent them from having to enter shelter.
  2. Develop creative housing solutions unique to the individual such as family mediation, roommate agreements, rental agreements, repayment agreements, verbal and/or written agreements between family members, conversations with probation officers, landlords, etc.
  3. Coordinate services such as local and long distance transportation for clients who have identified an alternative housing location.
  4. Maintain excellent communications among all parties (shelters, other homeless services providers, partner agencies, and first responders) to operate a high functioning shelter diversion program.
  5. Establish a written care or treatment plan with the client and family to enable the client to live with maximum possible independence.
  6. Provides assessment, eligibility screening, case planning, counseling, housing advocacy, tenant education, follow up and case management services to families accepted into rapid re-housing.
  7. Engages rapid re-housing clients in independent living and employment skills training as needed to develop a plan for housing stability and self-sufficiency.
  8. Ensures through on-going or time specific case management services as defined by the needs of the households enrolled in the program, that households a.) Access needed resources and services to maintain their housing b.) Receive support for problem solving, household management, life skills training, money management, and crisis intervention. 
  9. Assist in locating apartment units for households, procuring and updating the apartment leases with assistance from the Community Services Management Team as needed.
  10. Determine eligibility and provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
  11. Create and maintain confidential case records. Meet or exceed all program, funding source, and/or accreditation standards related to case documentation.
  12. Possess the ability to work independently and as part of a team, bringing concerns or challenges regarding complex issues to the attention of direct supervisor.

Requirements

  1. Bachelor’s degree in social work, psychology, or related human services field is required for this position. One (1) year of experience providing case management services is required for this position. A high school diploma with four (4) years of experience in a related field may substitute for bachelor’s degree. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. Experience in program specific population is preferred.
  3. Experience working with low income and people experiencing homelessness.
  4. Ability to work independently and to respond quickly and appropriately in times of crisis.
  5. Ability to relate well to individuals with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
  6. Ability to work complex cases and problem solve.
  7. Ability to work effectively and constructively with persons from all ethnic, racial, and social backgrounds and with co-workers and other community agencies.
  8. Passage of complete background clearance, Tuberculosis (TB) Test, and any other required testing or clearances.
  9. Catholic Charities, Diocese of Joliet, has adopted and follows the Codes of Ethics established by Catholic Charities USA, the National Association of Social Workers and the American Psychological Association. As an employee of the Agency you are expected to support these codes to the extent doing so is consistent with and allowed by applicable law.
  10. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
