General Responsibilities

  1. Operate as a member of the program team.
  2. Responsible for program paperwork including receipt transmittals and hotel payment check requests. 
  3. Conduct comprehensive needs assessments and provide coordination of services to assist identified population to gain access to and receive needed services.
  4. Monitor ongoing client services, advocate as needed for assigned client caseload.
  5. Provide services in an office or field based setting. 
  6. Act as agency ambassador in communities served.

Essential Functions

  1. Provide case management services in an office or field-based setting.
  2. Travel between Daybreak Shelter locations—Daybreak Center in Joliet, and hotels in Will, Kendall, and Grundy Counties.
  3. Following program guidelines, conduct comprehensive needs assessments & determine eligibility for services including state, federal, and local programs.
  4. Establish, if indicated, a written care or treatment plan with the client and family to enable the client to live with maximum possible independence.
  5. Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
  6. Has the ability to handle more complex cases; works with clients who may have multiple challenges.
  7. Complete monitoring and follow up with client as required by program or funding source. Track and submit appropriate receipts and other fiscal documents to support program spending.
  8. Create and maintain confidential case records. Meet or exceed all program, funding source, and/or accreditation standards related to case documentation.
  9. Assist in assuring data quality standards for program case records, both in data files and in HMIS.
  10. Complete all agency, program and other required reporting or documentation in an accurate manner according to required timeframes.
  11. Maintain a cooperative working relationship with service providers and community organizations to meet client needs.
  12. Conduct educational presentations for professional and community groups as assigned.
  13. Possess the ability to work independently and as part of a team, bringing concerns or challenges regarding complex issues to the attention of direct supervisor.
  14. Coordinate work schedule for adequate program coverage and utilize agency time and resources effectively. 
  15. Attend training, staff and team meetings. Complete any recertification or annual training requirements.

Requirements

  1. Bachelor’s degree in social work, psychology, or related field with one (1) year of experience providing case management services is required for this position a high school diploma with five (5) years of experience in a related field may substitute for bachelor’s degree; preferred is a Master’s degree with preference in areas of social work and a minimum of three (3) months of experience. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. Experience working with low income and people experiencing homelessness.
  3. Availability to work flexible days and hours as needed.
  4. Ability to work independently and to respond quickly and appropriately in times of crisis.
  5. Ability to relate well to individuals with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
  6. Ability to work complex cases and problem solve.
  7. Ability to work effectively and constructively persons from all ethnic, racial, and social backgrounds and with co-workers and other community agencies.
  8. Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
  9. Passage of complete background clearance, Tuberculosis (TB) Test, and any other required testing or clearances.
  10. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
