Downtown Women's Center is Hiring a Manager - Health Program Near Los Angeles, CA

Summary
Reporting to the Director of Health and Wellness, the Health Program Manager manages DWC’s health program and initiatives to support members’ outreach, enrollment and retention efforts. This individual is responsible for multiple grant projects, supervising staff, budgets and reporting. The Health Program Manager works collaboratively with DWC staff on training to enhance the enrollment services of our health programs and improve health care access and coverage for patients. This manager will oversee a team made up of a Benefits Specialist, Community Health Workers and Peer Specialist. In addition, this position will ensure the success of the Community Health Worker program. The ideal candidate will provide strategic vision, oversight, and supervision to a team of professional staff to enhance the health and wellness services offered at Downtown Women’s Center.
Essential Functions
  • Provide oversight of DWC’s Community Health Workers program and provide reporting and documentation as required.
  • Collaborate with the Workforce Development Department and medical professionals to develop Community Health Workers training curriculum.
  • Hire, train, and supervise a team of part-time Community Health Workers
  • Create and coordinate outreach materials for community health programming and for health program services
  • Provide education on various topics related to health resources, access to health benefits, and clinical resources in the community
  • Collaborate with homeless service provider agencies, community partners, public health departments, and federally qualified heath centers to coordinate health services such as vaccine education and access, health screenings and health workshops
  • Maintain database of services provided through the Health Program: HIV/STD testing, vaccine clinics, Recovery Groups, and locations for obtaining appointments
  • Facilitate feedback sessions with stakeholders and program participants to identify strategies for peer engagement
  • Work to effectively to resolve individual health barriers through timely follow-up, advocacy, relationships, and collaboration with DWC staff and other community service providers including hospitals, insurance providers and other medical providers
  • Provide crisis intervention to DWC participants
  • Conduct outreach to encourage client access to health & wellness services
  • Maintain accurate and timely records of activities, case management notes, and services provided to each client
  • Provide oversight of funding and grant requirements associated with health and wellness programs and compile information in collaboration with DWC’s Development team
  • Support the development of data systems, measurable outcomes and the implementation of ongoing program evaluation to ensure adherence to reporting requirements
  • Provide regular, written reports to the Director of Health and Wellness and additional reports as assigned
  • Offer supervision and guidance to interns and volunteers in the provision of services to clients as needed
  • Perform other duties as assigned to support the mission of the organization

Supervisory Responsibility
This position will supervise a team made of Health and Wellness staff that can include a Benefits Specialist, Health Program Peer Specialists, and Community Health Workers
Work Environment
This job operates in a Drop In Day Center/Health Clinic setting and in the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines and requires.

While performing the duties of this job, the employee is regularly required to talk and listen. Specific vision abilities required by this job include close vision and ability to adjust focus. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary and the physical ability to conduct community/street outreach.
Position Type and Expected Hours of Work
This is a full-time, exempt position. Days and hours of work are Monday through Friday, 8:00 a.m. to 4:30 p.m. with flexibility to work evenings and weekends when necessary.
Travel
Regular travel throughout Los Angeles County is required in this position.

Required Education and Experience
  • MSW, Masters in Marriage and Family Therapy or any postgraduate degree in Behavioral Health, preferred
  • Experience working as a medical case manager/patient navigator/care coordinator with homeless, low-income and/or disabled individuals and knowledge of the medical and the public benefits system preferred
  • Experience providing outreach with homeless, low-income and/or disabled individuals preferred
  • Experience working with peer workers with lived experience of homelessness preferred
Required Knowledge/Skills/Position Qualifications
  • Knowledge of the complexity of poverty, women’s homelessness, health issues, mental illness, trauma, substance abuse and older adults
  • Ability to problem solve and demonstrate an understanding of complex systems
  • Experience working in a medical, mental health, or public health setting
  • Excellent active listening and crisis intervention skills with the ability to exercise and model good judgment
  • Ability to manage multiple tasks and priorities; proficiency in balancing direct service responsibilities with effective record keeping
  • Ability to work well independently and with multiple teams while meeting the needs of both the clients and the organization
  • Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required
  • Strong interpersonal, written, and oral communication skills; a proven ability to communicate with diverse audiences
  • Oral and written fluency in both Spanish and English is strongly preferred
  • Current valid California state driver’s license, insurance, and vehicle with good driving record
  • Cultural humility.
  • Must be a CA resident and live no more than 80 miles from DWC; must be available to work in person at DWC and surrounding locations. Must provide documentation of a negative TB test result within the last twelve months, on first day of hire.
  • You will be required to be up to date on your vaccinations against COVID-19 and provide documentation. If you are fully vaccinated and not yet eligible to receive a booster dose, then you must receive the booster dose within 30 days of becoming eligible and eligibility begins 5 months after receiving final primary dose. If you are fully vaccinated and have received a booster dose, you are considered up to date with your vaccinations. Unvaccinated individuals must receive their 1st dose before their first day of hire. Partially vaccinated (for 2-dose series) individuals must receive their 2nd dose within 30 days of being hired. The second dose of a two-dose vaccine series is due 4 weeks after the primary dose.
Pay Range
$70,928/yearly
