Salary: $10,319.14 - $12,743.18 Monthly

The Office of Diversion and Reentry within the Los Angeles County Department of Health Services is looking for a Health Program Manager I (HPMI) to join the ODR Team and oversee the ODR Housing, Maternal Health, and Rapid Rehousing Programs. Reporting to ODR’s Deputy Director, The HPM 1 will supervise three management staff within the ODR Housing programs who oversee the following program areas: pre-release/court, post-release/community clinical consultation, and referral, access, and data (RAD).

The Health Program Manager will act as the Director of ODR’s Criminal Court Diversion Programs, the largest of which is the ODR Housing Program. The ODR Housing Program is focused on the release and subsequent treatment, support, and housing for persons with serious mental health needs being diverted from LA County Jails. Following release from custody, clients in the ODR Housing program are referred to a community based Intensive Case Management Services (ICMS) provider. The ICMS providers assist clients with accessing interim housing, treatment, following through on court ordered requirements, and eventually moving into and maintaining permanent supportive housing. The MHCS role will train, support and, supervise psychiatric social workers whose responsibilities include providing clinical consultation to the contracted CBOs in support of our shared clients.

The HPMI will serve as a primary liaison to, and collaborator with, project funders and program partners, involving matters of project implementation and performance, contracts, budgets, billing, program reports, and evaluation. 

Currently, ODR has a hybrid work schedule with a combination of workdays in the office, in the field and remote. This may change at discretion of DHS.

ESSENTIAL FUNCTIONS of the Health Program Manager I include, but are not limited to:

  • Plans, organizes, assigns, and coordinates the work of a team analysts serving as technical experts and consultants to management in major health service areas. 
  • Directs program planning, implementation, and management of ODR’s homeless and housing programs, including interim housing, residential mental health clinical-treatment housing, enriched residential care (ERC), rapid rehousing, and permanent supportive housing. 
  • Manages projects that serve high acuity clients with serious mental illness, and histories of homelessness, that are diverted from county jail to community-based services and housing. 
  • Develops policies and procedures, sets programmatic benchmarks, and collects and analyzes outcomes data. 
  • Provides regular oversight of referral and access system, housing case management services, employment services, rental subsidy administration, housing location and placement, and program intake and exit supporting ODR Housing clients including P3/P4 referrals. 
  • Directs the implementation and day-to-day operations of ICMS, permanent and interim housing projects, oversees the budget, and prepares project reports including expenditure reports, service utilization, performance data, and program outcomes. 
  • Has immediate responsibility for the orientation, training, development, and evaluation of employees supervised. 
  • Reviews work for thoroughness, soundness of recommendations, and compliance with applicable standards, policies, procedures, and regulations.
  •  Supervises and participates in studies of complex and difficult problem areas.
  • Analyzes project operations and outcomes and makes recommendations to improve effectiveness and efficiency. 
  • Negotiates the procurement of ICMS, interim and permanent housing, acute/subacute bed contractors supporting all ODR programs.
  •  Leads contract monitoring activities including annual administrative and programmatic monitoring and invoice processing. 
  • Create work plans, establish, and monitor progress toward goals, review work, and directly monitors the performance of analysts on this team on tasks including program and quality assurance monitoring, billing submission, invoice review and payment, data management and reporting, outcome tracking, preparation of board letters, client service prioritization and referral, clinical case conferencing, and analysis of program efficacy.
  • Monitors deliverables from all partners and identifies potential data collection roadblocks and identifies solutions.
  •  Monitors data quality and implements data improvement procedures as needed. 
  • Collects all demographic and programmatic data and tracks all participants for quality assurance purposes.
  • Provide administrative oversight, project management, and ongoing technical support to the ODR Clinical Team and contracted community-based organizations.
  • Monitor, research, and analyze operational processes, impact of program policies, resource utilization, and program effectiveness, with a focus on continuous quality improvement. 
  • Make recommendations for resource distribution, staffing needs, process improvements, cost savings, and program enhancements.
  • Consult with senior management on strategic planning for the department, including evaluation of growth opportunities, fiscal sustainability, projecting programmatic, operational, and administrative needs, and organizing and implementing new initiatives. 
  • Provides regular programmatic updates to ODR, Steering Committee and member departments, contracted agencies, Board of Supervisors, and stakeholder groups. 
  • Confers with administrators and representatives of other jurisdictions and agencies to discuss new and ongoing programs and exchange information. 
  • Coordinates and collaborates with all project partners (e.g., ODR, participating departments, contracted agencies, stakeholder groups) to ensure all elements of the projects operate in the most efficient and effective manner. Project partners, including, but not limited to: DHS Housing for Health, Department of Mental Health, Los Angeles County Board of Supervisor Offices, Los Angeles County Probation, Los Angeles Superior Court, Office of the Public Defender, District Attorney, Alternatives for Incarceration (ATI), Jail Closure Implementation Team (JCIT), Medicaid Managed Care Health Plans, Community Based Organizations, and private philanthropy.
  • Develop, document, and implement department-wide policies and procedures for ODR’s Clinical Court Diversion Programs. 
  • Ensures alignment with department standards, program regulations, and project contracts.
  • Develops and monitors Memorandums of Understanding (MOU) with partners, facilitates regular meetings to manage project implementation and oversight, and works with support service providers to ensure seamless and integrated systems of care. 
  • Researches and identifies philanthropic and government funding opportunities to sustain and/or expand the projects and prepares proposals to request funding. 
  • Research, advocate, and pursue funding for new and expansion projects. 
  • Develop, draft, and submit project proposals, including creating program design, drafting project budgets, developing goals and outcome measures.
  • In collaboration with subordinate staff and ODR leadership (including the Program Implementation Manager, ODR Medical Director and Director of ODR) this HPM I will implement new or expansion projects when awarded. 

JOB QUALIFICATIONS

Education/Experience

  • A Bachelor's degree or higher from an accredited college or university in a discipline related to the core business function of the department AND
  • Four (4) years in a highly responsible management experience in health care administrative operations for high-risk populations (e.g., homeless, justice-involved, mentally ill, substance use disorder or medically complex) - OR - (B) Researching, studying, and making recommendations to support health or social service programs or policy.
  • A Master's degree or higher from an accredited college or university in a discipline related to the core business function of the department (Social Work, Clinical Psychology, Marriage and Family Therapy, Public Health, Public Policy, etc), will be accepted for one year of the required experience.

Preferred Education/Experience

  • Master’s degree in Social work, Public Health, Public Policy, or related field strongly preferred.
  • Licensed as a mental health clinician (LCSW, MFT, LPCC, PsyD, etc). 
  • Ability to provide clinical supervision to license waived/pre-licensed mental health clinicians.
  • Social work or direct service experience in community-based programs strongly preferred.
  • Experience in program operations or program management. 
  • Ability to independently manage and implement program activities in coordination with ODR team. 
  • Demonstrated experience building collaboration and partnerships among diverse stakeholders, e.g., government, community organizations and businesses. 
  • Ability to coordinate day to day work among multidisciplinary teams (analysts, doctors, nurses, social workers, etc.) and service providers, including providing real-time problem solving and technical assistance as needed.
  • Strong synthesis, analysis and writing skills with ability to develop products, protocols, and reports.
  • Strong organizational skills, including an ability to manage multiple work projects simultaneously, track project details, and meet deadlines.
  • Strong technical skills with Microsoft excel and experience with database management (e.g., Electronic Health Record Systems) preferred. 
  • Ability to attend meetings, provide training, technical assistance, and other job-related duties in locations throughout Los Angeles County and have reliable transportation to carry out essential functions.

Certificates/Licenses/Clearances

  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Successfully clear the LA County Live Scan process.
  • Successfully clear the health clearance
