Dominion Care is Hiring a Senior Service Coordinator Near Richmond, VA

The Service Coordinator oversees service delivery, care coordination, and staff supervision in Waiver department. Supports overall quality of charts and documentation, provides training to direct reports with focus on continuing improvement and professional development, and represents TRACK core competencies. Support Program Manager with program operations and acts as Health and Safety representative for the local Waiver location(s). Provides mentorship to other Service Coordinators.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Coordinates the development and implementation of service plans, objectives and methods necessary for providing effective services to individuals.
  • Complete quarterlies for each individual served to summarize the service provided.
  • Acts as liaison with providers for process reporting and service planning.
  • Assists in admission process and discharge planning.
  • Participates in provider meetings, community meetings and other providers and program activities essential for the operation of an effective program.
  • Responsible for the development of the daily living program for each individual.
  • Manage staff to be assigned to individuals to include managing the professional development schedule, quality assurance, and corrective remediation.
  • Provide quality customer service to individuals served, their families, case managers, etc.
  • Supports Program Manager with operational responsibilities for the program such as facility maintenance oversight, health and safety representative for the location, and liaison with Quality Assurance and Person Centered Thinking coach to identify trainings to maintain and/or improve quality.
  • Mentors Service Coordinators and supports with onboarding.

QUALIFICATIONS

  • Education: A high school diploma or equivalent is usually required. Some positions may prefer or require a bachelor's degree in social work, psychology, human services, or a related field.
  •  Experience: Previous experience working in a related field, such as caregiving, direct support, or case management, is often necessary. The required years of experience may vary depending on the employer and the specific role.
  •  Knowledge: A strong understanding of relevant regulations, policies, and procedures related to in-home support services is essential. This includes knowledge of disability services, Medicaid regulations, and relevant state and federal laws.
  •  Communication Skills: Excellent communication skills, both verbal and written, are crucial for effectively interacting with clients, their families, other service providers, and community resources.
  •  Organizational Skills: Service coordinators must be highly organized to manage caseloads, maintain accurate documentation, and coordinate services effectively.
  •  Problem-Solving Abilities: The ability to assess needs, identify resources, and develop solutions to address challenges is essential for service coordinators working in in-home support services.
  •  Empathy and Compassion: A compassionate and empathetic approach to working with individuals with disabilities and their families is vital for building trust and rapport.
  •  Cultural Competence: Sensitivity to cultural differences and an understanding of how they may impact service delivery is important for providing inclusive and culturally competent support.
  •  Computer Skills: Proficiency in basic computer skills, including word processing, email, and data entry, is often required for documenting client information and communicating with team members.
