Diversus Health is Hiring a Behavioral Health Intake Assessment Specialist Near Colorado, CO

Job Summary:

As an essential member of the Same Day Access team, the Behavioral Health Intake Assessment Specialist conducts interviews, assessments, screenings, and questionnaires with clients and families seeking behavioral health treatment. Works with the patient access team, same-day access counselors, and referral team to interview clients to collect information needed to get clients into care on the same day they contact Diversus Health. Conduct assessments in-person or via telehealth, and ensures assessment data is entered into databases and electronic health records immediately. Outreaches to clients to schedule and conduct reassessments.

Duties include: contacts and outreaches to all SAMHSA-designated clients via phone or in-person, schedules appointments for assessments, gathers data, conducts assessments and follow-up assessments through an interview process, and completes data entry in multiple systems, including electronic health record systems. Conducts data entry from assessments completed by other team members..


Essential Functions:

  • Conducts assessment interviews in-person or via telehealth through an interview process.
  • Data entry into multiple electronic record-keeping systems, including electronic health record systems.
  • Verifies the accuracy of all information and data entered into the record-keeping systems.
  • Maintains a consistently calm and positive demeanor while interacting with clients by phone, video, or in person.
  • Ability to adapt to a consistently changing fast-paced environment that requires excellent multi-tasking skills.
  • Seeks and maintains efficiencies for Diversus Health’s patients’ access processes
  • Must be flexible with the business needs and report daily with a can-do approach.
  • Develops and maintains strong internal/ external working relationships and communication.
  • Attends and completes all required job-specific training and meetings as designated.
  • Maintains a positive work environment.
  • Performs all other duties as assigned.

Knowledge/Skills/Abilities:

  • Preferred experience working in a behavioral health or healthcare setting.
  • Preferred knowledge of trauma-informed care practices.
  • Excellent data entry skills; Type 25 w.p.m.
  • Excellent computer skills, with experience using programs within Microsoft Office, including Word, Outlook, and Excel.
  • Electronic Health Records experience preferred.
  • Excellent verbal and written communication skills.
  • Experience working in a team environment.
  • Excellent organizational skills.
  • Ability to maintain sensitive and confidential information.
  • Strong ability to interact successfully with a variety of individuals.
  • Proven work history in a fast-paced environment with competing priorities and the expectation to multi-task.
