Summary of Duties and Responsibilities:
Under the direction of the Director of Home and Community Based Services (HCBS), the Recovery Manager coordinates all related Specialized Recovery Service Program services received by an individual and assists the individual in gaining access to needed Medicaid State Plan and 1915(i) services, as well as medical, social, educational, and other resources, regardless of funding source.
Essential Functions:
Minimum Job Qualifications:
Bachelor’s degree in Social Work, Counseling, Psychology, Registered Nurse, or similar field required. A minimum of three years post degree experience working with individuals with severe and persistent mental illness (SPMI). Training in administering ANSA, person-centered planning, evaluating HCBS living arrangements, 42 CFR part 2 confidentiality of alcohol and drug abuse patient records, incident reporting and meets state conflict of interest standards.
Applicant must demonstrate the knowledge and skills necessary to provide the level care management/assessment appropriate to the scope of program responsibilities.
Experience with any of the following is desirable: Individuals with severe and persistent mental health or those with end stage renal failure or other chronic conditions.
Applicant must possess a valid driver’s license and proof of automobile insurance.
Must also pass a criminal background check and drug screen.
