Direct Access Counseling and Supportive Services, LLC is Hiring a Behavioral Health Care Manager Near Charlotte, NC

Behavioral Health Care Manager - Make up to $70,000 per year from home 

Immediate full-time, part-time, and flex positions available

DIRECT ACCESS is looking for BSW's, MSW's, and LPN’s, who want to work from the comfort of their home and enjoy a flexible schedule providing Behavioral Health Integration (BHI). 

Responsibilities include:

  • Interview and enroll patients in the BHI services
  • Coordinate patient care needs
  • Communicate with patients care team, which may include physicians, family members, and other care providers
  • Document patient communication
  • Create and maintain a comprehensive, proprietary patient care plan
  • Utilize the care plan to guide patient interaction and direction
  • Utilize chronic condition specific scripts to guide conversation
  • Review individual care plans and make monthly phone calls to CCM/BHI enrolled patients
  • Other duties as needed
