Job Summary:

Accountable for achieving effective and measurable clinical, financial, and functional outcomes. Expert clinician who works in partnership with the multidisciplinary team to coordinate development, implementation, and evaluation of a holistic plan of care for the patient and/or family across the continuum of care. Utilizing evidence-based practice, focuses on larger populations for the purpose of improving clinical performance, patient care outcomes and resource utilization. Functions as a leader supervising projects in their area(s) of responsibility. Promotes a safe environment, performs all job responsibilities in a safe manner, and maintains clinical and professional competency.

Essential Functions:

  • Utilizing evidence-based practice, focuses on larger population for the purpose of improving clinical performance, patient care outcomes, and resource utilization.
  • Analyzes and evaluate clinical data as it relates to clinical outcomes and resource utilization.
  • Develops process improvement goals and strategies and organizes and lead process improvement teams.
  • Develops standards of care/clinical guidelines/protocols in collaboration with multidisciplinary groups.
  • Serves as a clinical and systems expert resource.
  • Leads and participate on hospital teams, committees, etc.
  • Interprets information and activities for other healthcare providers.
  • Communicates information/outcomes regarding system issues and/or patient populations to appropriate individual or groups.
  • Provides direction to facilitate/coordinate the progress of patient groups along the continuum of care to achieve expected outcomes based on established guidelines or protocols.
  • Focuses the activities of the healthcare team on the achievement of optimal outcomes, resource utilization, clinical expertise, and improvement strategies.
  • Incorporates population specific knowledge into practice area.
  • Provides education regarding database, patient groups, outcomes, resource utilization, clinical expertise and best practice strategies.
  • Presents educational programs or assist in the development of educational materials related to clinical area of expertise or case management/improvement efforts.
  • Serves as mentor for healthcare providers.
  • Participates in ongoing evidence-based practice projects and studies.

Physical Requirements:

Works in an office type setting, extensive walking throughout the facility. Prolonged periods of sitting, reviewing medical records, and documentation. Repetitive wrist motion and occasional lifting of up to 35 pounds. Requires frequent verbal and written communication in English. Intact sight and hearing with or without assistive devices is required. Additional department specific physical requirements may be identified for unique responsibilities within the department by Leadership.

Education, Experience and Certifications:

BSN required; Masters preferred. Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works. Two years’ experience required in Care Management at Atrium Health. Appropriate professional certification required within 3 years of hire date per departmental protocol. Obtain and maintain BLS certification. 

Clinical competence in disease management and case management principles. Must possess excellent interpersonal communication and negotiation skills, problem-solving skills, strong organizational and time management skills, and the ability to work independently and as a member of the care team. Requires demonstrated knowledge and proficiency in appropriate tools.
