We are seeking a skilled and compassionate Nurse Practitioner with experience in geriatric care to join our team. The ideal candidate will have a strong background in managing the health needs of elderly patients, particularly in nursing home environments. This role can be tailored to fit either part-time or full-time availability.

Key Responsibilities:

• Provide comprehensive primary and preventive care to elderly patients in nursing homes and other care facilities.
• Conduct thorough assessments, including medical history, physical exams, and diagnostic tests.
• Develop, implement, and manage personalized care plans for geriatric patients.
• Prescribe medications and therapies as necessary, ensuring patients' understanding and adherence.
• Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and therapists, to optimize patient care.
• Monitor and manage chronic conditions, such as diabetes, hypertension, and dementia, adjusting treatment plans as needed.
• Educate patients and their families about health conditions, treatments, and preventive care strategies.
• Participate in the development and implementation of quality improvement initiatives within the facility.
• Maintain accurate and timely documentation in compliance with regulatory standards.

Qualifications:

• Master’s or Doctoral degree in Nursing from an accredited program.
• Current and valid Nurse Practitioner license in New York.
• Certification as a Geriatric Nurse Practitioner or Nurse Practitioner.
• Minimum of [3] years of experience working with geriatric patients, preferably in a nursing home or similar setting.
• Strong clinical skills with the ability to manage complex and chronic health conditions.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
