We are seeking a highly skilled and compassionate Geriatric Nurse Practitioner to join our team during the week at our nursing home facility. The ideal candidate will have extensive experience in geriatric care and a commitment to providing exceptional healthcare services to our elderly residents. Responsibilities:

• Conduct comprehensive health assessments for elderly residents, including physical examinations, medical history reviews, and medication management.
• Develop personalized care plans tailored to each resident's unique needs and preferences.
• Collaborate with interdisciplinary healthcare teams, including physicians, nurses, therapists, and social workers, to ensure coordinated and holistic care delivery.
• Provide direct patient care, including but not limited to wound care, chronic disease management, and acute illness treatment.
• Educate residents and their families about health conditions, treatment options, and preventative measures to promote optimal health and well-being.
• Monitor resident's health status and response to treatment interventions, adjusting care plans as necessary to ensure optimal outcomes.
• Assist in end-of-life care planning and support for residents and their families.
• Adhere to all regulatory guidelines and standards of practice in geriatric care.
• Maintain accurate and up-to-date medical records for all residents.
• Participate in ongoing professional development activities to stay abreast of advancements in geriatric healthcare.

Qualifications:

• Master's degree in Nursing from an accredited institution.
• Current licensure as a Nurse Practitioner in the state of [insert state].
• Certification as a Geriatric Nurse Practitioner preferred.
• Minimum of at least 3 years of experience in geriatric care, preferably in a nursing home or long-term care setting.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a team environment.
• Empathy and compassion for elderly individuals and their families.

• Flexibility to work weekends on a regular basis.
